【摘要】：目的探討乳房外Paget病的臨床及組織病理學特點。方法收集16例乳房外Paget病患者臨床及組織病理學資料,并復習相關文獻。結(jié)果本組男性15例,女性1例,平均年齡70.4歲。皮損主要發(fā)生于頂泌汗腺分布較豐富的區(qū)域,如陰囊、陰莖、肛周、會陰等部位,少數(shù)發(fā)生于腋窩、腹股溝。臨床主要表現(xiàn)為境界清楚的浸潤性紅斑或斑塊,表面滲出、糜爛和結(jié)痂。典型的組織病理特點為表皮內(nèi)散在或成巢狀分布的Paget細胞,免疫表型:CK7、CEA、GCDFP-15等均陽性,CK5/6、CK20、S-100蛋白、HMB-45等均陰性。結(jié)論乳房外Paget病是一種少見的惡性程度較低的腫瘤,好發(fā)于老年男性,該病誤診率高,早期臨床易誤診為濕疹等疾病。典型的組織病理學表現(xiàn)結(jié)合免疫表型可明確診斷,對于疑似病例應及早行組織病理學檢查有利于減少臨床誤診,手術(shù)切除為首選治療方法,若切除不完整極易復發(fā),需長期隨訪。
[Abstract]:Objective to investigate the clinical and histopathological features of extramammary Paget's disease. Methods the clinical and histopathological data of 16 patients with extramammary Paget's disease were collected and the relevant literatures were reviewed. The average age was 70.4 years. The skin lesions mainly occurred in areas with abundant distribution of sweat glands, such as scrotum, penis, perianal, perineum and so on, and a few occurred in armpit and groin. The clinical manifestations were infiltrating erythema or plaque with clear boundary. Surface exudation, erosion and scab. Typical histopathological features are scattered or nestled Paget cells in the epidermis. The immunophenotype of CK7, CEA, GCDFP-15, et al were all negative. Conclusion extramammary Paget's disease is a rare tumor with low malignancy, and is prone to occur in elderly men, and the misdiagnosis rate of the disease is high. The typical histopathological manifestations combined with immunophenotype can be clearly diagnosed. Early histopathological examination should be used to reduce clinical misdiagnosis and surgical resection is the first treatment method. If excision is incomplete and easy to relapse, long-term follow-up is needed.
