【摘要】：目的:通過(guò)對(duì)比光動(dòng)力聯(lián)合多西環(huán)素和單用多西環(huán)素治療頭部膿腫性穿掘性毛囊周?chē)?PCAS)的臨床療效,探討10%5-氨基酮戊酸光動(dòng)力療法(ALA-PDT)聯(lián)合多西環(huán)素治療PCAS的有效性及安全性。方法:根據(jù)入選原則將收集的40例PCAS患者隨機(jī)分為聯(lián)合治療組(20例)和對(duì)照組(20例)。對(duì)照組:予以服用鹽酸多西環(huán)素腸溶膠囊0.1g/次,2次/日,連續(xù)服用30日。聯(lián)合治療組:服用鹽酸多西環(huán)素腸溶膠囊,方法同對(duì)照組,并同時(shí)予以ALA-PDT治療,每10日1次,連續(xù)3次。兩組均以30日為一療程,分別在第10、20、30日進(jìn)行療效判斷及記錄不良反應(yīng),并于療程結(jié)束后第1、2月進(jìn)行隨訪(fǎng)。結(jié)果:兩組患者的基線(xiàn)資料無(wú)顯著的統(tǒng)計(jì)學(xué)差異(P0.05),兩組患者在療程的第10、20、30日的Sartorius評(píng)分組間評(píng)分比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);聯(lián)合治療組及對(duì)照組在療程第10日的顯效率分別為5%、0,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);聯(lián)合治療組在療程第20、30日的顯效率分別為31.6%、51.9%,對(duì)照組在療程第20、30日的顯效率分別為5%、20%,組間比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05);聯(lián)合治療組在療程第10、20日治療的總有效率分別為63.2%、84.2%,對(duì)照組在療程第10、20日治療的總有效率分別為20.0%、50%,組間效率差異有統(tǒng)計(jì)學(xué)意義(P0.05);在療程30日兩組總有效率分別為100%、95%,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。光動(dòng)力治療PCAS不良反應(yīng)主要表現(xiàn)為水腫性紅斑、燒灼感、瘙癢、色素沉著等,未見(jiàn)嚴(yán)重不良反應(yīng)。結(jié)論:PDT聯(lián)合多西環(huán)素和單用多西環(huán)素治療PCAS均有較好療效;聯(lián)合治療療效好,能顯著提高患者治療顯效率。10%ALA-PDT治療PCAS不良反應(yīng)小。
[Abstract]:Objective: to compare the clinical effects of photodynamic therapy combined with doxycycline and doxycycline alone in the treatment of head abscess perforating perifollicular perifolliculitis (PCAS). To investigate the efficacy and safety of 105-aminolevulinic acid photodynamic therapy combined with doxycycline in the treatment of PCAS. Methods: according to the principle of inclusion, 40 patients with PCAS were randomly divided into two groups: the combined treatment group (n = 20) and the control group (n = 20). Group B: take doxycycline hydrochloride enteric capsule 0.1g / time twice a day, The combined treatment group was treated with doxycycline hydrochloride enteric-coated capsule in the same way as the control group and was treated with ALA-PDT once every 10th for 3 consecutive times. Both groups were treated with 30th as a course of treatment. The therapeutic effects were evaluated and adverse reactions were recorded at the 10th and 20th, 30th, respectively. The results showed that there was no significant difference in baseline data between the two groups (P 0.05), and there was a significant difference between the two groups in the Sartorius score in 30th. The effective rates in the treatment group and the control group on the 10th of the course of treatment were 50.The difference was not statistically significant (P 0.05); the marked effect rate in the combined treatment group was 31.6% and 51.9% in the 20th and 30th courses of treatment, respectively, while in the control group on the 20th and 30th courses of treatment, the effective rate was 50.20%, respectively. The difference between the two groups was poor. The total effective rates in the combined treatment group were 63.2% and 84.2% respectively in the 10th course of treatment and in 20th, respectively. In the control group, the total effective rates were 20.0% and 50% on the 10th course of treatment and 20th, respectively, and the difference between the two groups was statistically significant (P 0.05); in 30th, the total effective rate of the treatment group was higher than that of the control group (P < 0.05), and the total effective rate of the treatment group was significantly higher than that of the control group (P < 0.05). The total effective rates of the group were 100% and 95%, respectively. There was no significant difference between the two groups (P 0.05). The main adverse effects of photodynamic therapy on PCAS were edema erythema. There was no serious adverse reaction. Conclusion the combination of doxycycline and doxycycline has good effect on PCAS. Can significantly improve the effective rate of treatment. 10 ALA-PDT treatment of PCAS adverse reactions.
