【摘要】：目的：觀察玉屏銀屑方延長(zhǎng)尋常型復(fù)發(fā)型銀屑病緩解期的療效及預(yù)防干預(yù)用藥前后患者外周血T細(xì)胞亞群水平的變化，初步探討玉屏銀屑方延長(zhǎng)尋常型復(fù)發(fā)型銀屑病緩解期的作用機(jī)理。 方法：收集病例共87例，隨機(jī)分為玉屏銀屑方組30例（A組）、轉(zhuǎn)移因子膠囊組27例（B組）和停藥組30例（C組）。A組和B組治療4周后停藥，觀察一年；C組入組即停藥與A組做平行對(duì)照，觀察一年。A組隨機(jī)抽取20人，于入組前和治療4周后進(jìn)行外周血T細(xì)胞亞群檢測(cè)，并與20個(gè)正常人T細(xì)胞亞群做對(duì)照分析。對(duì)所有入組患者隨訪觀察一年，主要觀察評(píng)估PASI、瘙癢程度、QOL、復(fù)發(fā)時(shí)間和不良事件。 結(jié)果：T細(xì)胞亞群檢測(cè)：A組患者治療前，患者外周血T淋巴細(xì)胞亞群低于正常范圍，治療后T細(xì)胞亞群水平相比治療前改善顯著。 臨床觀察結(jié)果分析：復(fù)發(fā)率比較，A組復(fù)發(fā)率明顯低于C組。復(fù)發(fā)患者PASI評(píng)分比較，A組復(fù)發(fā)患者PASI評(píng)分顯著低于C組，且A組復(fù)發(fā)患者PASI評(píng)分增加幅度明顯低于C組。瘙癢程度比較，A組復(fù)發(fā)患者瘙癢程度顯著低于C組，且A組復(fù)發(fā)患者瘙癢程度的增加幅度明顯低于C組。入組患者QOL評(píng)分比較，A組患者結(jié)束觀察研究時(shí)QOL評(píng)分非常顯著低于C組，并進(jìn)一步提示A組患者前后QOL評(píng)分減少數(shù)值非常顯著大于C組。緩解時(shí)間比較，A組復(fù)發(fā)患者本次緩解期顯著比C組延長(zhǎng)，并進(jìn)一步提示A組復(fù)發(fā)患者緩解期延長(zhǎng)時(shí)間非常顯著高于C組。 A組與B組以上臨床觀察結(jié)果分析各項(xiàng)比較均無(wú)顯著差異，各項(xiàng)比較均基本持平。 結(jié)論：玉屏銀屑方能夠顯著延長(zhǎng)尋常型復(fù)發(fā)型銀屑病患者的緩解期，能夠改善T細(xì)胞的免疫功能和免疫平衡。
[Abstract]:Objective: to observe the curative effect of Yuping Yinfeng recipe in prolonging the remission period of recurrent psoriasis vulgaris and the changes of T cell subsets in peripheral blood of patients with Psoriasis vulgaris before and after preventive intervention. Objective: to explore the mechanism of Yuping Yinfeng recipe in prolonging the remission period of recurrent psoriasis vulgaris. Methods: 87 cases were randomly divided into group A (30 cases), group A (27 cases), group B (27 cases), group B (30 cases), group C (30 cases) and group B (30 cases). The T lymphocyte subsets of peripheral blood were detected before and after 4 weeks of treatment in group A, and 20 patients in group A were randomly selected for one year. All the patients were followed up for one year to evaluate PASI, the degree of pruritus and QOLs, recurrence time and adverse events. Results before treatment, T lymphocyte subsets in peripheral blood of patients in group A were lower than those in normal range, and the level of T lymphocyte subsets was significantly improved after treatment. Analysis of clinical observation results: the recurrence rate in group A was significantly lower than that in group C, the PASI score in group A was significantly lower than that in group C, and the PASI score in group A was significantly lower than that in group C. The degree of pruritus in group A was significantly lower than that in group C, and the degree of pruritus in group A was significantly lower than that in group C. The degree of pruritus in group A was significantly lower than that in group C. the QOL score of group A was significantly lower than that of group C at the end of the study. It was further suggested that the decrease of QOL score in group A was significantly larger than that in group C. the remission time of group A was significantly longer than that of group C, and the remission time of group A was significantly longer than that of group C. It was further suggested that the duration of remission in group A was significantly longer than that in group C. There was no significant difference in clinical observation results between group A and group B. Conclusion: Yuping Yinfeng prescription can significantly prolong the remission period of patients with recurrent psoriasis vulgaris and improve the immune function and balance of T cells.
