【摘要】：目的探討腎上腺皮質(zhì)癌(ACC)皮膚轉(zhuǎn)移的臨床表現(xiàn)、病理學(xué)特點(diǎn)及診斷,提高對本病的認(rèn)識。方法分析1例ACC皮膚轉(zhuǎn)移的臨床表現(xiàn)、組織病理及免疫組織化學(xué),復(fù)習(xí)相關(guān)文獻(xiàn)并進(jìn)行討論。結(jié)果患者右臀部腫物進(jìn)行性增大伴輕度疼痛1月余。ACC行腹腔鏡下左腎上腺區(qū)腫瘤切除術(shù)2年。皮膚科情況:右臀部為一直徑約5cm近圓形暗紅色腫塊,局部皮溫略高,質(zhì)地硬,境界較清楚,壓痛明顯。腫塊組織病理示:真皮內(nèi)可見境界清楚的腫瘤細(xì)胞團(tuán)塊,細(xì)胞核大,圓形或橢圓形,呈不典型性,有核絲分裂相,胞漿豐富呈嗜酸性或淡染;免疫組織化學(xué)染色示:腫瘤細(xì)胞Synaptophysin,Vimentin強(qiáng)陽性;Calretinin,Inhibin陽性;Chromogranin A,SMA,CD10,CK,S100,HMB45,Melan A,Desmin,CEA均陰性。結(jié)合免疫組織化學(xué)和病史,符合ACC皮膚轉(zhuǎn)移。結(jié)論 ACC皮膚轉(zhuǎn)移國內(nèi)尚未見報道,國外罕有報道。組織病理及免疫組織化學(xué)對本病的診斷及鑒別診斷有重要作用。
[Abstract]:Objective to investigate the clinical manifestations, pathological features and diagnosis of skin metastasis of adrenal cortical carcinoma (ACC), and to improve the understanding of this disease. Methods the clinical manifestations of 1 case with ACC skin metastasis were analyzed. Histopathology and immunohistochemistry. Results the patients with progressive enlargement of the right buttocks with mild pain in January underwent laparoscopic resection of the left adrenal area tumor for 2 years. Dermatology:. The right buttocks were a dark red mass about 5 cm in diameter. The local skin temperature was slightly higher, the texture was hard, the boundary was clear, and the tenderness was obvious. Histopathology of the tumor showed that the tumor cell mass with clear boundary could be seen in the dermis, the nucleus was large, round or oval, and it was atypical. There was mitotic phase and the cytoplasm was rich in eosinophilic or light staining. Immunohistochemical staining showed that the tumor cells were strongly positive for Synaptophysinia sinustii vimentin (Vimentin). Calretinin Inhibin positive; Chromogranin A,SMA,CD10,CK,S100,HMB45,Melan A,Desmin. CEA was negative. Combined with immunohistochemistry and history, it was consistent with ACC skin metastasis. Conclusion ACC skin metastasis has not been reported in China. It is rarely reported abroad. Histopathology and immunohistochemistry play an important role in the diagnosis and differential diagnosis of this disease.
