非酮癥糖尿病患者糖化血紅蛋白與紅細(xì)胞分布寬度變異系數(shù)和甲狀腺過氧化物酶抗體陽性率的相關(guān)性及其變化的影響因素研究

【摘要】:目的分析非酮癥糖尿病患者糖化血紅蛋白(HbA_(1c))與紅細(xì)胞分布寬度變異系數(shù)(RDW-CV)和甲狀腺過氧化物酶抗體(TPOAb)陽性率的相關(guān)性,并探討HbA_(1c)變化的影響因素。方法選取2014年3月—2016年4月在益陽市中心醫(yī)院內(nèi)分泌科住院的符合納入標(biāo)準(zhǔn)的非酮癥糖尿病患者413例。依據(jù)患者HbA_(1c)進(jìn)行分組:HbA_(1c)≤7.5%組(A組,n=128)、7.5%HbA_(1c)10.0%組(B組,n=190)、HbA_(1c)≥10.0%組(C組,n=95)。比較3組患者性別、年齡、病程、體質(zhì)指數(shù)(BMI)、RDW-CV、低密度脂蛋白(LDL)、總膽固醇(TC)、24 h尿微量清蛋白(24 h UAE)、C反應(yīng)蛋白(CRP)、空腹C肽(FCP)、餐后2 h C肽(PCP)、游離三碘甲狀腺原氨酸(FT3)、游離甲狀腺素(FT4)、促甲狀腺激素(TSH)、谷氨酸脫羥酶抗體(GADA)陽性率、TPOAb陽性率,分析HbA_(1c)與上述指標(biāo)的相關(guān)性及影響患者HbA_(1c)變化的獨(dú)立危險(xiǎn)因素。結(jié)果 B組、C組患者病程長于A組,RDW-CV大于A組,TSH及TPOAb陽性率高于A組(P0.05);C組患者病程長于B組,RDW-CV大于B組,TSH、TPOAb陽性率高于B組(P0.05)。Spearman秩相關(guān)分析結(jié)果顯示,患者HbA_(1c)與病程、RDW-CV、TPOAb陽性率、TSH呈正相關(guān)(rs值分別為0.681、0.630、0.432、0.438,P值分別為0.001、0.001、0.001、0.001)。RDW-CV[OR=2.024,95%CI(1.237,4.103)]、TPOAb陽性率[OR=1.243,95%CI(1.134,1.548)]、TSH[OR=1.814,95%CI(1.567,3.234)]是非酮癥糖尿病患者HbA_(1c)變化的獨(dú)立危險(xiǎn)因素(P0.05)。結(jié)論非酮癥糖尿病患者HbA_(1c)與RDW-CV、TPOAb陽性率呈正相關(guān),且RDW-CV、TPOAb陽性率是患者HbA_(1c)變化的獨(dú)立危險(xiǎn)因素,因此,血糖控制越差,紅細(xì)胞形態(tài)改變越明顯,免疫性甲狀腺疾病發(fā)生的可能性越大。
