皮膚糖基化終產(chǎn)物無創(chuàng)檢測篩查2型糖尿病的臨床應(yīng)用

【摘要】:目的評估皮膚AGE無創(chuàng)檢測技術(shù)在糖尿病篩查中的應(yīng)用價值。方法選取208例T2DM患者和119名健康體檢者行體征檢查、生化檢測,采用基于AGE光學(xué)檢測技術(shù)的糖尿病無創(chuàng)檢測儀(DM Scan)對受試者上臂皮膚組織中的AGE進(jìn)行無創(chuàng)檢測,采用受試者工作特征(ROC)曲線評價其敏感性和特異性。結(jié)果≤30歲、30~50歲和≥50歲糖尿病患者AGE均高于健康受試者[(56.2±4.1)vs(71.3±8.4)AU、(62.0±5.9)vs(70.6±6.2)AU、(62.5±6.9)vs(75.8±8.4)AU](P0.01)。Pearson相關(guān)分析顯示,AGE與FPG(r=0.417,P0.01)和HbA_1c(r=0.500,P0.01)相關(guān)。多元回歸分析顯示,AGE值與糖尿病發(fā)病風(fēng)險密切相關(guān)[OR1.322,95%CI(1.228,1.424),P0.01]。DM Scan檢測皮膚AGE的ROC為0.941[95%CI(0.917,0.965)],AGE切點(diǎn)65.00AU時對應(yīng)的敏感性為88.5%,特異性為89.9%。結(jié)論 DM Scan檢測皮膚AGE篩查T2DM有較高的敏感性和特異性,有望成為一種新的無創(chuàng)糖尿病篩查工具。
