【摘要】：目的:探討左甲狀腺素對(duì)橋本氏甲狀腺炎患者輔助性T淋巴細(xì)胞的影響。方法:選擇51例橋本氏甲狀腺炎患者作為左甲狀腺素組,另同期選擇30名健康人作為對(duì)照組。左甲狀腺素組患者給予左旋甲狀腺素鈉片25~50μg,口服,每日1次,根據(jù)甲狀腺功能調(diào)整用藥劑量,待甲狀腺功能正常后,以此劑量維持治療,連用90 d。觀察左甲狀腺素組患者治療前后游離三碘甲狀腺原氨酸(FT3)、游離甲狀腺素(FT4)、促甲狀腺素(TSH)水平和Th17細(xì)胞占外周血CD4~+T細(xì)胞百分率(Th17/CD4~+),并與對(duì)照組進(jìn)行比較,分析Th17/CD4~+與甲狀腺功能的相關(guān)性。結(jié)果:治療后,左甲狀腺素組患者Th17/CD4~+顯著低于同組治療前,但高于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);TSH水平顯著低于同組治療前,FT3、FT4水平均顯著高于同組治療前,差異均有統(tǒng)計(jì)學(xué)意義(P0.05),但與對(duì)照組比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。左甲狀腺素組患者Th17/CD4~+與甲狀腺功能具有正相關(guān)性(r=0.815,P0.05)。結(jié)論:左甲狀腺素可顯著降低橋本氏甲狀腺炎患者的Th17/CD4~+、TSH水平,升高FT3、FT4水平,且Th17/CD4~+與橋本氏甲狀腺炎的發(fā)生發(fā)展具有相關(guān)性。
[Abstract]:Objective: to investigate the effect of levothyroxine on helper T lymphocytes in patients with Hashimoto's thyroiditis. Methods: 51 patients with Hashimoto's thyroiditis were selected as left thyroxine group and 30 healthy people as control group. The patients in the levothyroxine group were given levothyroxine sodium tablets 25 ~ 50 渭 g oral once a day. The dosage was adjusted according to the thyroid function. After the thyroid function was normal, the dosage was maintained for 90 days. The levels of free triiodothyronine (FT3), free thyroxine (FT4), thyrotropin (TSH) and the percentage of Th17 cells in peripheral blood CD4~ T cells (Th17/CD4~) were observed before and after treatment in left thyroxine group. Compared with the control group, the correlation between Th17/CD4~ and thyroid function was analyzed. Results: after treatment, the Th17/CD4~ of left thyroxine group was significantly lower than that of the same group before treatment, but higher than that of the control group, the difference was statistically significant (P0.05). The level of TSH was significantly lower than that of the same group before treatment, and the level of FT3,FT4 was significantly higher than that of the same group before treatment (P 0.05), but there was no significant difference compared with the control group (P 0.05).
