【摘要】：目的:回顧性分析了130例甲狀腺不確定結(jié)節(jié)的臨床評(píng)估和預(yù)后,以及組織細(xì)胞學(xué)檢測(cè)結(jié)果,為甲狀腺結(jié)節(jié)的治療提供合理的治療方法。方法:根據(jù)英國甲狀腺協(xié)會(huì)2014年分類,回顧性分析了2011年1月至2014年12月130例甲狀腺不確定結(jié)節(jié)(Thy 3)的細(xì)胞學(xué)檢查。將結(jié)節(jié)分為Thy 3a(非典型特征)和Thy 3f(濾泡病變)兩類。比較評(píng)估術(shù)前生物化學(xué)檢查、彩色多普勒超聲檢查(US)、半定量彈性超聲成像(USE)、超聲造影(CEUS)、細(xì)胞學(xué)、組織學(xué)以及分子生物學(xué)突變分析等結(jié)果。結(jié)論:Thy 3結(jié)節(jié)的惡性腫瘤發(fā)生率較低,且在Thy 3a和Thy 3f之間沒有顯著差異。使用基于組織學(xué)的截取值作為參考可以降低采用手術(shù)的治療方案。在陰性突變的Thy 3a和Thy 3f結(jié)節(jié)中,當(dāng)不是所有的儀器檢測(cè)結(jié)果都疑是陽性時(shí),組織學(xué)觀察應(yīng)該作為首選。
[Abstract]:Objective: To retrospectively analyze the clinical evaluation and prognosis of 130 cases of thyroid indeterminate nodules, and to provide a reasonable treatment for the treatment of thyroid nodules. Methods: According to the 2014 classification of the British thyroid association, the cytological examination of the thyroid-indeterminate nodules (Thy 3) from January 2011 to December 2014 was analyzed retrospectively. The nodules were divided into Thy 3a (atypical) and Thy 3f (follicular lesions). The results of preoperative biochemical examination, color Doppler ultrasonic examination (US), semi-quantitative elastic ultrasound imaging (USE), ultrasonic contrast (CEUS), cytology, histology and mutation analysis of molecular biology were compared. Conclusion: The incidence of malignant tumor of Thy 3 nodules is low, and there is no significant difference between Thy 3a and Thy 3f. The use of histology-based intercept values can be used as a reference to reduce the treatment regimen using the procedure. In the negative mutated Thy 3a and Thy 3f nodules, the histological observation should be preferred when not all of the instrument test results were suspected to be positive.
