【摘要】：目的探究低谷濃度他克莫司(cTAC)降低活體肝移植術(shù)后糖尿病(PTDM)發(fā)病風(fēng)險(xiǎn)的安全性與可行性。方法回顧性分析2005年6月至2014年9月,我中心符合納入標(biāo)準(zhǔn)的158例活體肝移植患者資料。定義平均cTAC為診斷PTDM當(dāng)年的cTAC平均值,或未診斷PTDM患者最近一年的cTAC平均值。運(yùn)用受試者工作特征曲線(ROC曲線)確定平均cTAC臨界值。結(jié)果 34例(21.5%)患者診斷PTDM。PTDM組1、5、10年移植物生存率及總體生存率均低于非PTDM組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。預(yù)測(cè)PTDM的平均cTAC臨界值為5.9ng/mL。低cTAC組(≤5.9ng/mL)術(shù)后血脂異常、PTDM、心腦血管疾病與中重度感染發(fā)生率低于高cTAC組(5.9ng/mL),差異有統(tǒng)計(jì)學(xué)意義(P0.05);然而兩組間急、慢性排斥反應(yīng)發(fā)生率的差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論活體肝移植術(shù)后6個(gè)月后,維持cTAC≤5.9ng/mL是安全有益的。
[Abstract]:Objective to investigate the safety and feasibility of low concentration tacrolimus (cTAC) in reducing the risk of diabetic (PTDM) after living donor liver transplantation (LLT). Methods from June 2005 to September 2014, 158 living-related liver transplantation patients in our center were analyzed retrospectively. The mean cTAC was defined as the mean value of cTAC in the year of diagnosis of PTDM, or the mean value of cTAC in the last year of undiagnosed PTDM. The mean cTAC critical value was determined by using the receiver operating characteristic curve (ROC curve). Results in 34 patients (21.5%), the 1, 5, 10-year graft survival rate and overall survival rate in PTDM.PTDM group were lower than those in non-PTDM group (P0.05).
