大氣污染對(duì)糖尿病前期及健康人群的心肺及代謝系統(tǒng)健康效應(yīng)的前瞻性定組研究

【摘要】:大氣污染物是心肺系統(tǒng)疾病的重要風(fēng)險(xiǎn)因子,然而其對(duì)代謝系統(tǒng)疾病(如糖尿病等)的影響尚不明確.我國(guó)大氣污染形勢(shì)嚴(yán)峻,糖尿病患病人口基數(shù)大,對(duì)應(yīng)疾病負(fù)擔(dān)嚴(yán)重,因而研究大氣污染物暴露對(duì)糖尿病發(fā)病及病理進(jìn)程影響機(jī)制十分必要.現(xiàn)有的研究證據(jù)對(duì)回答心血管異常疾病患者對(duì)于大氣污染物是否易感,以及大氣污染物是否會(huì)通過(guò)特定生物機(jī)制通路而加劇血糖代謝異常病理進(jìn)程等問(wèn)題仍存在不足.本研究組基于已有研究結(jié)果以及以往研究局限性,建立針對(duì)上述科學(xué)問(wèn)題的前瞻性定組研究(SCOPE).本研究針對(duì)糖尿病前期及健康對(duì)照人群,通過(guò)1年間4次重復(fù)隨訪采樣分析大氣污染物的健康效應(yīng),對(duì)環(huán)境暴露及健康終點(diǎn)進(jìn)行綜合評(píng)價(jià),并比較兩組人群對(duì)大氣污染響應(yīng)的效應(yīng)差異.健康終點(diǎn)評(píng)價(jià)方面,通過(guò)生物標(biāo)志物測(cè)量血糖代謝異常、胰島素抵抗、內(nèi)皮功能損傷、系統(tǒng)性炎癥等多種生物機(jī)制通路損傷.環(huán)境暴露方面,對(duì)顆粒物組分及粒徑進(jìn)行精細(xì)測(cè)量,同時(shí)加入個(gè)體水平暴露評(píng)價(jià).本研究有助于深入探索大氣污染物對(duì)心血管代謝異常人群的健康效應(yīng),及其對(duì)應(yīng)的機(jī)制通路.
[Abstract]:Air pollutants are important risk factors for cardiopulmonary diseases, but their effects on metabolic diseases such as diabetes are unclear. The situation of air pollution in China is severe, the population base of diabetes mellitus is large and the burden of disease is serious. Therefore, it is necessary to study the influence mechanism of air pollutant exposure on the pathogenesis and pathological process of diabetes mellitus. The existing research evidence is still insufficient to answer the questions whether the patients with cardiovascular disorders are susceptible to atmospheric pollutants and whether the atmospheric pollutants will aggravate the pathological process of abnormal blood glucose metabolism through a specific biological mechanism pathway. Based on the results of previous studies and the limitations of previous studies, the study group has established a prospective grouping study (SCOPE). For the above scientific problems. In this study, the health effects of atmospheric pollutants were analyzed by 4 repeated follow-up samples in pre-diabetic and healthy control population, and the environmental exposure and health endpoint were evaluated comprehensively. The responses of the two groups to air pollution were compared. In the evaluation of health endpoints, the biomarkers were used to measure the dysfunction of blood glucose metabolism, insulin resistance, endothelial dysfunction, systemic inflammation and so on. In the aspect of environmental exposure, the composition and particle size of particulate matter were measured carefully, and the individual level exposure evaluation was added at the same time. This study is helpful to further explore the health effects of atmospheric pollutants on people with abnormal cardiovascular metabolism and the corresponding mechanisms.
