【摘要】：目的對(duì)比分析艾滋病病毒(HIV)抗原抗體聯(lián)檢電化學(xué)發(fā)光法(ECL)反應(yīng)性結(jié)果與蛋白印跡法(WB)確證實(shí)驗(yàn)檢測(cè)結(jié)果,找出ECL診斷HIV陽(yáng)性病人的閾值,探討HIV檢測(cè)新策略。方法收集鄭州大學(xué)第一附屬醫(yī)院2012年4月至2016年2月646例病人復(fù)篩ECL及HIV確證試驗(yàn)結(jié)果,對(duì)23例WB不確定結(jié)果進(jìn)行隨訪確診,回顧性分析兩組檢驗(yàn)資料;采用受試者工作特征曲線(ROC)評(píng)估ECL檢測(cè)HIV抗體的能力。結(jié)果 ECL檢測(cè)方法的總陽(yáng)性率為59.44%(384/646),ECL診斷HIV抗體的ROC曲線下面積為0.990,cutoff值32.17,靈敏度97.10%,特異度96.20%;以cutoff值為界值,COI≥32.17的病人中97.14%為真陽(yáng)性,COI≥200時(shí),確證實(shí)驗(yàn)陽(yáng)性率為100%,COI32.17的病人中96.23%為假陽(yáng)性。結(jié)論 ECL檢測(cè)值COI越大,WB的陽(yáng)性率越高,該結(jié)果可為采用酶聯(lián)免疫試驗(yàn)檢測(cè)HIV抗體的檢測(cè)策略提供重要的參考依據(jù)。
[Abstract]:Objective to compare and analyze the results of (ECL) reactivity of (HIV) antigen-antibody and Western blot (WB), to find out the threshold value of ECL in the diagnosis of HIV positive patients, and to explore a new method for HIV detection. Methods from April 2012 to February 2016, 646 patients were confirmed by ECL and HIV in the first affiliated Hospital of Zhengzhou University. 23 cases of WB uncertainty were followed up and the data of the two groups were analyzed retrospectively. The ability of ECL to detect HIV antibodies was evaluated by using the operating characteristic curve (ROC). Results the total positive rate of ECL was 59.44% (384 / 646), ECL).
