糖尿病性耳聾聽力變化特點及其發(fā)病機(jī)制

【摘要】:第一部分糖尿病性耳聾聽力變化特點目的:探討自發(fā)性糖尿病鼠聽力變化特點。方法:清潔級雄性,7周齡Zucker Diabetes Fat(ZDF)大鼠根據(jù)糖尿病發(fā)病與否分為發(fā)生糖尿病的ZDF模型鼠(fa/fa)(實驗組)10只,不發(fā)生糖尿病的ZDF對照鼠(fa/+)(對照組)5只。分別于2、3、4、5、6月大小時測大鼠的體重、空腹血糖、短聲誘發(fā)聽性腦干反應(yīng)(Click-ABR)、短純音誘發(fā)聽性腦干反應(yīng)(tb-ABR)、畸變產(chǎn)物耳聲發(fā)射(DPOAEs)。結(jié)果:實驗鼠在第2月齡內(nèi)12小時禁食不禁飲測空腹血糖在正常范圍,第3月齡時實驗鼠的空腹血糖已達(dá)糖尿病標(biāo)準(zhǔn)并維持在較高水平;對照組大鼠血糖一直保持在正常范圍。根據(jù)測得空腹血糖值大小實驗組空腹血糖值上升相對較快的鼠(5只)在4月齡時tb-ABR在8k Hz上的閾值較其2月齡時閾值升高(p0.05);Click-ABR 90d BSPLⅠ波潛伏期、Ⅰ-Ⅲ間期較在2月齡時延長(p0.05),4月后Ⅰ波潛伏期、Ⅰ-Ⅲ間期開始縮短。實驗組空腹血糖上升相對較慢鼠在5月齡時tb-ABR在8k Hz、16k Hz、32k Hz的閾值較2月齡時閾值升高(p0.05),以32k Hz聽力損失最嚴(yán)重,Click-ABR90d BSPLⅠ波及Ⅰ-Ⅲ波潛伏期出現(xiàn)較明顯延長(p0.05),5月后Ⅰ波潛伏期、Ⅰ-Ⅲ間期開始縮短。對照組大鼠實驗前后聽力無明顯變化(p0.05)。結(jié)論:肥胖ZDF糖尿病大鼠發(fā)病前期聽力無明顯變化,糖尿病鼠發(fā)病穩(wěn)定后1到2個月(即4、5月齡)開始出現(xiàn)較明顯的聽力損害;糖尿病聽力損失的發(fā)病進(jìn)展速度也許與血糖升高的快慢有關(guān);糖尿病鼠高頻聽力損失較重,提示臨床糖尿病人應(yīng)當(dāng)監(jiān)測高頻聽力變化為主。第二部分糖尿病鼠內(nèi)耳結(jié)構(gòu)的變化特點目的:分析自發(fā)性糖尿病鼠內(nèi)耳結(jié)構(gòu)變化特點,為糖尿病的發(fā)病機(jī)制提供基礎(chǔ)支持。方法:通過石蠟切片HE染色,耳蝸基底膜鋪片的免疫熒光染色及掃描電鏡觀察糖尿病鼠耳蝸的基本結(jié)構(gòu)、內(nèi)外毛細(xì)胞及其纖毛排列、血管紋的變化情況。結(jié)果:石蠟切片HE染色提示ZDF糖尿病鼠血管紋較對照鼠變薄,基底膜鋪片及免疫熒光染色觀察毛細(xì)胞及其纖毛排列無明顯變化。掃描電鏡觀察提示ZDF糖尿病發(fā)病鼠與對照鼠頂回均有部分外毛細(xì)胞纖毛及其表皮板缺失,缺失部分被上皮樣細(xì)胞替代,ZDF糖尿病發(fā)病鼠與對照鼠比較,其頂回內(nèi)毛細(xì)胞纖毛較對照鼠有明顯缺失,表皮板尚完整。ZDF糖尿病發(fā)病鼠與對照鼠中回及底回內(nèi)、外毛細(xì)胞纖毛無明顯脫落。結(jié)論:糖尿病鼠內(nèi)耳結(jié)構(gòu)免疫組化及電鏡上主要以血管紋及內(nèi)毛細(xì)胞損傷為主;內(nèi)耳大體結(jié)構(gòu)未見明顯改變。綜合糖尿病鼠聽力檢測及耳蝸形態(tài)結(jié)構(gòu)改變,糖尿病性聽力損失特點可能主要表現(xiàn)為隱性聽力損失(HHL)。故臨床上建議糖尿病病人隨訪耳純音測聽及言語識別率(特別是噪音下言語識別率)檢測,同時注意檢測耳蝸電圖的變化,以便于更好的觀察糖尿病對聽力的損傷情況。
