【摘要】：目的觀察雷珠單抗聯(lián)合玻璃體切割術(shù)治療糖尿病視網(wǎng)膜病變合并新生血管性青光眼的臨床效果。方法回顧性分析糖尿病視網(wǎng)膜病變合并新生血管性青光眼并行雷珠單抗聯(lián)合玻璃體切割術(shù)的16例(16眼)患者的臨床資料。術(shù)后隨訪6個月,對比觀察手術(shù)前后眼壓、最佳矯正視力、虹膜新生血管、房角開閉情況等,并記錄并發(fā)癥情況。結(jié)果術(shù)前患眼眼壓為(42.54±16.52)mmH g(1 kP a=7.5 mmH g)。玻璃體內(nèi)注射雷珠單抗后1 d眼壓為(38.78±11.38)mmH g,與術(shù)前相比差異無統(tǒng)計學(xué)意義(P0.05);玻璃體切割術(shù)后1周眼壓(45.30±12.20)mmH g,與術(shù)前相比差異無統(tǒng)計學(xué)意義(P0.05);術(shù)后1個月、3個月、6個月眼壓分別為(27.56±19.38)mmH g、(20.49±18.66)mmH g、(19.79±16.53)mmH g,與術(shù)前相比差異均有統(tǒng)計學(xué)意義(均為P0.05)。隨訪結(jié)束時,10眼(62.5%)虹膜新生血管完全消退,6眼(37.5%)虹膜新生血管明顯減少。結(jié)論雷珠單抗聯(lián)合玻璃體切割術(shù)治療糖尿病視網(wǎng)膜病變合并新生血管性青光眼,可有效降低眼壓,輕度提高視力,術(shù)后并發(fā)癥少。
[Abstract]:Objective to observe the clinical effect of Leizhu McAb combined with vitrectomy in the treatment of diabetic retinopathy with neovascular glaucoma. Methods the clinical data of 16 patients (16 eyes) with diabetic retinopathy complicated with neovascular glaucoma and vitrectomy were retrospectively analyzed. The intraocular pressure, best corrected visual acuity, iris neovascularization, atrial angle opening and closing were observed and the complications were recorded. Results preoperative IOP was (42.54 鹵16.52) mmH g (1 kP a 7.5 mmH g). The intravitreal intraocular pressure (IOP) was (38.78 鹵11.38) mmH g,) after injection of Lei Zhu McAb (P0.05). Intraocular pressure (IOP) 1 week after vitrectomy (45.30 鹵12.20) mmH g,) was not significantly different from that before vitrectomy (P0.05). The intraocular pressure (IOP) was (27.56 鹵19.38) mmH g, (20.49 鹵18.66) mmH g, (19.79 鹵16.53) mmH g,) at 1 month, 3 months and 6 months after operation respectively, which was significantly different from that before operation (P0.05). Iris neovascularization disappeared completely in 10 eyes (62.5%) and iris neovascularization decreased in 6 eyes (37.5%) at the end of follow-up. Conclusion Threzumab combined with vitrectomy in the treatment of diabetic retinopathy with neovascular glaucoma can effectively reduce intraocular pressure, slightly improve visual acuity, and have less postoperative complications.
