【摘要】：目的分析老年2型糖尿病(T2DM)患者血清胰島素(FINS)水平與骨密度(BMD)及骨代謝指標(biāo)的相關(guān)性。方法選取老年T2DM患者(T2DM組)128例、同期健康體檢者(對(duì)照組)50例,測定兩組腰椎L2~L4、股骨頸、Ward's三角及大轉(zhuǎn)子(Troch)BMD,比較兩組骨質(zhì)疏松(OP)發(fā)生率,并檢測兩組血清FINS、總Ⅰ型前膠原氨基端延長肽(TPⅠNP)、Ⅰ型膠原羧基端肽β特殊序列(CTX-β)水平,采用Pearson線性相關(guān)法分析各指標(biāo)之間的關(guān)系。結(jié)果與對(duì)照組比較,T2DM組L2~L4、股骨頸、Ward's三角及Troch BMD均降低,OP發(fā)生率升高(P均0.05);與對(duì)照組比較,T2DM組血清FINS降低,TPⅠNP、CTX-β水平均升高(P均0.05)。相關(guān)性分析顯示,各部位BMD均與血清FINS水平呈正相關(guān),股骨頸、Ward's三角BMD與血清TPⅠNP、CTX-β水平呈負(fù)相關(guān)(P均0.05),且血清INS水平與TPⅠNP、CTX-β水平呈負(fù)相關(guān)(P均0.05)。結(jié)論老年T2DM患者血清FINS水平與BMD呈正相關(guān),與血清TPⅠNP、CTX-β水平呈負(fù)相關(guān)。
