強(qiáng)直性脊柱炎骶髂關(guān)節(jié)MRI平掃與X線平片、CT對(duì)照分析

【摘要】:目的:探究MRI平掃與X線平 CT檢查對(duì)早期強(qiáng)直性脊柱炎(AS)骶髂關(guān)節(jié)病變的診斷情況。方法:回顧性分析廣西醫(yī)科大學(xué)第一附屬醫(yī)院臨床確診的23例早期強(qiáng)直性脊柱炎病例的臨床和影像資料,3名中高級(jí)醫(yī)生通過對(duì)三種影像學(xué)圖除資料對(duì)照分析,探究早期AS骶髂關(guān)節(jié)改變的特點(diǎn),運(yùn)用卡方檢驗(yàn)分析三種檢查方法對(duì)骶髂關(guān)節(jié)病變檢出的臨床應(yīng)用價(jià)值。結(jié)果:X線平除僅能確診9例,CT確診14例,MRI確診19例患者,23例患者M(jìn)RI表現(xiàn)均有異常,三種影像學(xué)檢查方法在診斷早期AS和檢出骶髂關(guān)節(jié)病變分級(jí)的差異有顯著意義(P㩳0.05)。早期AS的骶髂關(guān)節(jié)MRI主要表現(xiàn)為關(guān)節(jié)旁骨髓水腫、滑膜炎和關(guān)節(jié)軟骨破壞等。結(jié):MRI對(duì)早期強(qiáng)直性脊柱炎的診斷有明顯優(yōu)勢(shì),結(jié)合臨床資料及實(shí)驗(yàn)室檢查,可做出早期診斷。
[Abstract]:Objective: to investigate the diagnosis of sacroiliac joint lesions of early ankylosing spondylitis by MRI plain scan and X-ray plain CT. Methods: the clinical and imaging data of 23 cases of early ankylosing spondylitis diagnosed in the first affiliated Hospital of Guangxi Medical University were analyzed retrospectively. To explore the characteristics of sacroiliac joint changes in early AS and to analyze the clinical value of three methods in detecting sacroiliac joint lesions by chi-square test. Results: only 9 cases were diagnosed by X-ray, 14 cases by CT, 19 cases by MRI, and 23 cases by MRI. There were significant differences among the three imaging methods in the diagnosis of early AS and the detection of sacroiliac joint lesions (P0. 05). The main manifestations of sacroiliac joint MRI in early AS were bone marrow edema, synovitis and articular cartilage destruction. Conclusion: MRI has obvious advantages in the diagnosis of early ankylosing spondylitis.
