【摘要】：目的確定吸煙對(duì)甲狀腺相關(guān)眼病(TAO)患者眶脂肪分化的影響。設(shè)計(jì)回顧性病例系列。研究對(duì)象2012年6-12月在上海長征醫(yī)院眼科行眶減壓術(shù)的TAO患者40例,男性27例,女性13例。方法 40例TAO患者中,21例吸煙、8例曾吸煙但已戒煙,5例被動(dòng)吸煙,6例非吸煙。運(yùn)用RT-PCR檢測所有患者眶脂肪CCAAT/增強(qiáng)結(jié)合蛋白(C/EBP)和過氧化物酶體增生物激活受體(PPARγ)mRNA表達(dá)量。用△Ct法進(jìn)行其各基因表達(dá)的相對(duì)定量。多組間的總體比較采用單因素方差分析,隨后用SNK-q檢驗(yàn)進(jìn)行兩兩比較。主要指標(biāo)C/EBP和PPARγmRNA表達(dá)量。結(jié)果吸煙組患者的C/EBP(12.04±4.26)和PPARγmRNA表達(dá)量(4.82±1.41)明顯高于非吸煙組(C/EBP:2.32±0.92;PPARγ:1.37±0.25)(P0.001)。同時(shí),曾吸煙組和被動(dòng)吸煙組的C/EBP(4.29±1.26,5.76±1.03)和PPARγmRNA表達(dá)量(2.91±1.23,3.22±0.50)對(duì)比非吸煙組的差異也具有統(tǒng)計(jì)學(xué)意義(曾吸煙組C/EBP P=0.039,PPARγP=0.047;被動(dòng)吸煙組C/EBP P=0.002,PPARγP=0.002)。吸煙指數(shù)≥100年支的TAO患者眶脂肪C/EBP(14.28±3.61)和PPARγmRNA(5.65±1.02)高于吸煙指數(shù)100年支的患者(C/EBP:8.42±2.30;PPARγ:3.48±0.74)(P均0.01)。結(jié)論眶脂肪C/EBP和PPARγmRNA的表達(dá)量與吸煙史有關(guān)。
[Abstract]:Objective to determine the effect of smoking on orbital adipose differentiation in patients with thyroid associated ophthalmopathy (TAO). Design retrospective case series. Participants 40 TAO patients, 27 males and 13 females, underwent orbital decompression in Shanghai long March Hospital from June to December 2012. Methods among 40 patients with TAO, 21 were smokers, 8 had smoked but had given up smoking, 5 were passive smokers and 6 were non-smokers. The expressions of CCAAT/ enhanced binding protein (C/EBP) and peroxisome proliferator-activated receptor (PPAR 緯) mRNA in orbital fat were detected by RT-PCR. The relative quantitative analysis of the gene expression was carried out by Ct method. Single factor analysis of variance (ANOVA) was used to compare the population of different groups, and then pairwise comparison was performed with SNK-q test. Main outcome measures: expression of C/EBP and PPAR 緯 mRNA. Results the expression of C/EBP and PPAR 緯 mRNA in smoking group (12.04 鹵4.26) was significantly higher than that in non-smoking group (C/EBP:2.32 鹵0.92) (1.37 鹵0.25) (P0.001).
