【摘要】：目的更新既往的有關(guān)的研究資料,使用循證醫(yī)學(xué)的方法,為進(jìn)一步系統(tǒng)的評(píng)價(jià)糖尿病(Diabetes mellitus, DM)和結(jié)直腸癌(Colorectal cancer, CRC)之間的發(fā)生風(fēng)險(xiǎn),以進(jìn)一步闡明糖尿病是否是促進(jìn)結(jié)直腸惡性腫瘤發(fā)生發(fā)展的相關(guān)風(fēng)險(xiǎn)的促進(jìn)要素之一,借以為糖尿病人群進(jìn)行結(jié)直腸惡性腫瘤的早期排查提供進(jìn)一步的臨床根據(jù),從而對(duì)能夠使糖尿病患者早期進(jìn)行惡性腫瘤的篩查,做到能夠早發(fā)現(xiàn)、早治療,從而使糖尿病病患的生存質(zhì)量得到改善。方法通過(guò)計(jì)算機(jī)網(wǎng)絡(luò)檢索1994年1月1日至2014年10月31日PubMed、 MEDLINE、EMBase、中國(guó)知網(wǎng)、維普、萬(wàn)方等中英文數(shù)據(jù)庫(kù)中公開發(fā)表的探討糖尿病與結(jié)直腸惡性腫瘤關(guān)系的大型隊(duì)列研究或者病例對(duì)照研究的文獻(xiàn),根據(jù)制定的統(tǒng)一的文獻(xiàn)納入以及剔除的標(biāo)準(zhǔn)進(jìn)行文獻(xiàn)的篩選、核對(duì)等,然后提取相關(guān)數(shù)據(jù)資料,最后通過(guò)采用Review Manager5.3統(tǒng)計(jì)學(xué)軟件完成Meta分析,并通過(guò)比值比(Odds ratio, OR)作為效應(yīng)量對(duì)篩選后的文獻(xiàn)數(shù)據(jù)進(jìn)行合并分析。結(jié)果經(jīng)過(guò)文獻(xiàn)的篩選、排除等步驟后納入研究的共計(jì)16篇,共計(jì)積累研究病例1723632例,其中研究組103753例,對(duì)照組1619879例。經(jīng)過(guò)Review Manager 5.3軟件進(jìn)行數(shù)據(jù)的處理后,Meta分析結(jié)果顯示：OR=1.38,95%可信區(qū)間(1.31,1.45),進(jìn)行合并效應(yīng)檢驗(yàn)后Z=12.52,P0.00001。數(shù)據(jù)結(jié)果提示糖尿病患者結(jié)直腸惡性腫瘤的發(fā)生率較對(duì)照組偏高,差異具有統(tǒng)計(jì)學(xué)的意義。結(jié)論結(jié)直腸惡性腫瘤的發(fā)生和糖尿病具有高度的關(guān)聯(lián)性,糖尿病是促進(jìn)患者結(jié)直腸惡性腫瘤的危險(xiǎn)因素之一。
[Abstract]:Objective to evaluate the risk of diabetes mellitus (Diabetes mellitus, DM) and colorectal cancer (Colorectal cancer, CRC) by updating previous relevant research data and using evidence-based medicine (EBM) methods to further evaluate the risk of diabetes mellitus (DM) and colorectal cancer (Colorectal cancer, CRC). To further clarify whether diabetes is one of the factors contributing to the risk associated with the development of colorectal malignancies, and to provide further clinical evidence for the early detection of colorectal malignancies in diabetics. It can make the patients with diabetes cancer screening early, so that early detection, early treatment, so as to improve the quality of life of patients with diabetes. Methods from January 1, 1994 to October 31, 2014, PubMed, MEDLINE,EMBase, China knowledge Network, Weip, was searched by computer network. The literature published in Wanfang and other Chinese and English databases on large-scale cohort studies or case-control studies on the relationship between diabetes mellitus and colorectal malignant tumors is screened according to the unified criteria for inclusion and elimination of the established documents. Finally, the Meta analysis was completed by using Review Manager5.3 software, and the selected literature data were combined by ratio (Odds ratio, OR) as the effect quantity. Results A total of 16 cases were included in the study, including 103753 cases in the study group and 1619879 cases in the control group. After processing the data by Review Manager 5.3, the results of Meta analysis showed that the confidence interval of OR=1.38,95% (1.31 鹵1.45), and the combination effect test, Z _ (12.52) (P _ (0.00001). The data showed that the incidence of colorectal malignant tumor in diabetic patients was higher than that in the control group, and the difference was statistically significant. Conclusion there is a high correlation between colorectal malignant tumor and diabetes mellitus. Diabetes is one of the risk factors to promote colorectal malignant tumor.
