老年糖尿病與遺忘型輕度認(rèn)知功能障礙的相關(guān)性

【摘要】:目的探討老年糖尿病與遺忘型輕度認(rèn)知功能障礙(a MCI)的相關(guān)性。方法采用前瞻性隨訪隊(duì)列研究設(shè)計(jì)。共納入符合研究目的且資料完整的老年對(duì)象416例,分析老年糖尿病患者不同臨床特征與a MCI的相關(guān)性。結(jié)果老年糖尿病能夠明顯增加a MCI[HR(95%CI):1.82(1.05~3.17),P=0.032]、單個(gè)認(rèn)知領(lǐng)域損傷a MCI[HR(95%CI):2.35(1.18~4.68),P=0.013]及多個(gè)認(rèn)知領(lǐng)域損傷a MCI[HR(95%CI):2.40(1.00~5.81),P=0.045]的發(fā)生風(fēng)險(xiǎn)。老年糖尿病更能引起男性多個(gè)認(rèn)知領(lǐng)域損傷a MCI的發(fā)生[HR(95%CI):3.60(1.04~12.45),P=0.032]。糖尿病患者患病年齡≤65[HR(95%CI):2.52(1.01~6.30),P=0.042]、血糖調(diào)控失衡[HR(95%CI):3.58(1.26~10.19),P=0.011]、口服降糖藥[HR(95%CI):2.23(1.06~4.72),P=0.032]、胰島素治療[HR(95%CI):2.87(1.06~7.78),P=0.032]、視網(wǎng)膜病變[HR(95%CI):3.41(1.01~11.49),P=0.036]及神經(jīng)病變[HR(95%CI):2.63(110~6.32),P=0.025]是a MCI的危險(xiǎn)因素。結(jié)論老年糖尿病能夠明顯增加a MCI發(fā)生風(fēng)險(xiǎn),男性患者發(fā)生a MCI的風(fēng)險(xiǎn)更高,尤其是多個(gè)認(rèn)知領(lǐng)域損傷a MCI,護(hù)理中應(yīng)更加關(guān)注男性糖尿病患者。
