2型糖尿病患者下肢血管病變與血清25羥維生素D及骨密度的相關(guān)性

【摘要】:目的探討2型糖尿病患者下肢血管病變與血清25羥維生素D及骨密度的相關(guān)性。方法選取2型糖尿病患者235例,按踝肱指數(shù)(ankle brachial index,ABI)(采用多普勒血流探測儀測定)的數(shù)值將患者分為兩組,分別為下肢血管病變組(PAD組,ABI0.9)115例和非下肢血管病變組(NPAD組,ABI≥0.9)120例。采用雙能X線骨密度儀進行腰椎(L1~4)、左側(cè)股骨頸(Neck)、大轉(zhuǎn)子(Troch)、華氏三角(Ward's)骨密度(bone mineral density,BMD)的測定。測定受試者血清25(OH)D的水平,并記錄所有受試者的臨床資料和生化指標(biāo)。結(jié)果 PAD組血清25(OH)D的平均水平低于NPAD組[(12.89±1.48)ng/mL vs(15.82±2.35)ng/mL,P0.05];PAD組的Neck、Troch、Ward's的BMD均分別低于NPAD組[(0.706±0.171)g/cm~2vs(0.823±0.133)g/cm~2,(0.501±0.111)g/cm~2vs(0.595±0.125)g/cm~2,(0.455±0.121)g/cm~2vs(0.524±0.210)g/cm~2,P0.05]。Pearson相關(guān)分析發(fā)現(xiàn),ABI與年齡、糖尿病病程、收縮壓(systolic blood pressure,SBP)、膽固醇(total cholesterol,TC)、低密度脂蛋白膽固醇(low-density lipoprotein,LDL-C)、糖化血紅蛋白(hemoglobin A1c,Hb A1c)呈負(fù)相關(guān),與25(OH)D及Neck、Troch、Ward's的BMD呈正相關(guān),差異有統(tǒng)計學(xué)意義(P0.05)。Logistic分析顯示:SBP、Hb A1c、TC、25(OH)D、Neck的BMD均為2型糖尿病患者發(fā)生下肢血管病變的危險因素。結(jié)論 2型糖尿病合并下肢血管病變患者往往同時伴有骨量減少或骨質(zhì)疏松,低水平的25(OH)D及低下肢骨密度可能共同參與了2型糖尿病患者下肢動脈病變的發(fā)生發(fā)展。
