抗C反應(yīng)蛋白單體抗體在系統(tǒng)性紅斑狼瘡中的作用

【摘要】：目的觀察系統(tǒng)性紅斑狼瘡患者中抗C反應(yīng)蛋白單體抗體水平的變化與疾病活動(dòng)度之間的關(guān)系。方法以天然的CRP為抗原,用酶聯(lián)免疫吸附方法對實(shí)驗(yàn)組30名系統(tǒng)性紅斑狼瘡患者、疾病對照組(30例其他風(fēng)濕性疾病,包括原發(fā)性干燥綜合征,類風(fēng)濕性關(guān)節(jié)炎,強(qiáng)直性脊柱炎,骨關(guān)節(jié)炎)和正常對照組(30例健康體檢組)血清中的抗mCRP抗體水平,同時(shí)檢測系統(tǒng)性紅斑狼瘡患者血清中的CRP、ESR、FET,記錄患者各項(xiàng)臨床表現(xiàn)及實(shí)驗(yàn)室指標(biāo),分析抗mCRP抗體與疾病活動(dòng)度之間的關(guān)系。結(jié)果系統(tǒng)性紅斑狼瘡患者的抗mCRP抗體水平明顯高于疾病對照組及健康對照組,差異有統(tǒng)計(jì)學(xué)意義(t=2.526,5.456,P均0.01);疾病對照組與健康對照組的抗mCRP抗體水平相比較,疾病對照組明顯高于健康對照組,差異有統(tǒng)計(jì)學(xué)意義(t=3.628,P0.01)。與穩(wěn)定組相比,輕、中、重度活動(dòng)組SLE患者抗mCRP抗體水平均明顯高于穩(wěn)定組(P均0.01)。差異均有統(tǒng)計(jì)學(xué)意義。且隨著疾病活動(dòng)度的增加,抗mCRP抗體水平也增加,與SLE患者的SLEDAI計(jì)分的高低呈正相關(guān)(r=0.238,P0.01)。抗mCRP抗體陽性組與陰性組相比較,陽性組ESR明顯增快(t=2.156,P0.01),抗mCRP抗體水平與ESR呈正相關(guān)(r=0.463,P0.01),差異有統(tǒng)計(jì)學(xué)意義;而在CRP、白細(xì)胞、血小板、C3、鐵蛋白等方面,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論抗mCRP抗體水平高低與SLE患者的ESR、SLEDAI評分呈正相關(guān)�？筸CRP抗體水平與SLE患者的疾病活動(dòng)度密切相關(guān),可以成為預(yù)測SLE患者的疾病活動(dòng)度指標(biāo)之一。
[Abstract]:Objective to investigate the relationship between the level of anti C reactive protein monomer antibody and disease activity in patients with systemic lupus erythematosus (SLE). Methods using natural CRP as antigen, 30 patients with systemic lupus erythematosus and 30 patients with other rheumatic diseases, including primary Sjogren's syndrome and rheumatoid arthritis, were studied by enzyme-linked immunosorbent assay (Elisa). Serum levels of anti mCRP antibody in ankylosing spondylitis, osteoarthritis) and normal control group (30 healthy controls) and CRP,ESR,FET, in serum of patients with systemic lupus erythematosus were also detected. The clinical manifestations and laboratory indexes were recorded and the relationship between anti-mCRP antibody and disease activity was analyzed. Results the level of anti mCRP antibody in patients with systemic lupus erythematosus was significantly higher than that in the disease control group and the healthy control group (P 0.01). Compared with the healthy control group, the level of anti mCRP antibody in the disease control group was significantly higher than that in the healthy control group (t = 3.628, P 0.01). Compared with the stable group, the level of anti mCRP antibody in the mild, moderate and severe active SLE group was significantly higher than that in the stable group (P0.01). The difference was statistically significant. With the increase of disease activity, the level of anti mCRP antibody also increased, which was positively correlated with the SLEDAI score of SLE patients (r = 0.238, P 0.01). Compared with the negative group, the ESR of the positive group increased significantly (t = 2.156, P0.01), and the level of anti-mCRP antibody was positively correlated with the ESR (r = 0.463, P 0.01), the difference was statistically significant. However, there was no significant difference in white blood cell, platelet, C _ 3, ferritin and so on in CRP, (P0.05). Conclusion the level of anti-mCRP antibody is positively correlated with the ESR,SLEDAI score of SLE patients. The level of anti-mCRP antibody is closely related to the disease activity of SLE patients and can be used as a predictor of disease activity in SLE patients.
