【摘要】：目的探討急診科就診高鈉血癥患者的一般臨床特征、癥狀與病死率的關(guān)系。方法對(duì)2014年4月到2016年4月到鄭州大學(xué)附屬洛陽(yáng)中心醫(yī)院急診科就診的高鈉血癥患者進(jìn)行了回顧性分析。239例患者血鈉離子(Na~+)145mmol/L,平均年齡(65.13±12.14)歲,平均Na~+水平為(157.71±9.42)mmol/L;根據(jù)血Na~+水平分為血Na~+145~160mmol/L組與血Na~+≥160mmol/L組;比較兩組患者的一般臨床特征、癥狀、疾病及病死率。結(jié)果血Na~+≥160mmol/L組老年癡呆癥發(fā)病率較血Na~+145~160mmol/L高(P=0.012),血肌酐、氯離子(Cl-)、血漿滲透壓水平均較高(P=0.014,P=0.001,P=0.004);血Na~+≥160mmol/L組患者的平均動(dòng)脈壓(MAP)和格拉斯哥昏迷評(píng)分(GCS)較血Na~+145~160mmol/L組低(P=0.001,P=0.000),脈搏較快(P=0.000);血Na~+≥160mmol/L組低血壓、氣促、消化道出血和神經(jīng)精神癥狀發(fā)生率較血Na~+145~160mmol/L組高(P0.01),脫水、肺炎、尿路感染的發(fā)病率較血Na~+145~160mmol/L組高(P0.01);Na~+≥160mmol/L組病死率明顯高于Na~+145~160mmol/L組(P=0.000)。多因素Logistic回歸分析顯示,低血壓、發(fā)熱、氣促、消化道出血及神經(jīng)精神癥狀與高鈉血癥患者病死率有相關(guān)性。結(jié)論嚴(yán)重高鈉血癥患者病死率較高,如患者合并低血壓、發(fā)熱、氣促、消化道出血及神經(jīng)精神癥狀等癥狀,臨床醫(yī)生需高度警惕,積極救治,盡力降低病死率。
