糖耐量正常人群OGTT 1h血糖與胰島素敏感性和胰島β細(xì)胞功能的關(guān)系

【摘要】:目的評(píng)價(jià)糖耐量正常(normal glucose tolerance,NGT)人群口服葡萄糖耐量試驗(yàn)(oral glucose tolerance test,OGTT)1 h血糖與胰島素敏感性和胰島β細(xì)胞功能的關(guān)系。方法收集101例NGT和76例糖耐量異常(impaired glucose tolerance,IGT)病人的臨床資料;測(cè)定OGTT各時(shí)間點(diǎn)的血漿葡萄糖和胰島素濃度;比較NGT 1 h血糖(1 hour postload plasma glucose,1-h PG)8.6 mmol/L組、NGT 1-h PG≥8.6 mmol/L組和IGT組的胰島素敏感性和胰島β細(xì)胞功能,分析OGTT 1-h PG與胰島素敏感性和胰島β細(xì)胞功能的關(guān)系。結(jié)果 NGT 1-h PG≥8.6 mmol/L組的空腹血糖、1-h PG、1-h胰島素及胰島素敏感指數(shù)(homeostasis model assessment of insulin resistance,HOMA-IR)均顯著高于NGT 1-h PG8.6 mmol/L組;NGT 1-h PG≥8.6 mmol/L組的Matsuda指數(shù)、胰島素分泌指數(shù)(ΔI3 0/ΔG3 0和ΔI6 0/ΔG6 0)均顯著低于NGT 1-h PG8.6 mmol/L組(P均0.05)。但NGT 1-h PG≥8.6mmol/L組的空腹血糖、空腹胰島素、1-h PG、1-h胰島素、HOMA-IR、Matsuda指數(shù)、ΔI3 0/ΔG3 0和ΔI6 0/ΔG6 0與IGT組比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P均0.05)。多元線性回歸分析顯示1-h PG與ΔI30/ΔG30、Matsuda指數(shù)呈線性負(fù)相關(guān)。結(jié)論在NGT人群中,OGTT 1-h PG≥8.6 mmol/L人群胰島素敏感性和胰島β細(xì)胞功能低于1-h PG8.6 mmol/L組,1-h PG與胰島素敏感性和胰島β細(xì)胞功能相關(guān)。提示在NGT人群中,1-h PG濃度可作為評(píng)價(jià)胰島素敏感性和胰島β細(xì)胞功能的簡(jiǎn)易指標(biāo)。
