初診2型糖尿病患者血清25-羥維生素D水平與胰島α細胞及β細胞功能的相關性

【摘要】:目的近年研究發(fā)現(xiàn),維生素D缺乏與糖尿病的發(fā)生和發(fā)展有一定關系。文中旨在初步探討初診2型糖尿病患者血清25-羥維生素D[25(OH)D]水平與胰島α細胞、β細胞功能的關系。方法選取2015年4月至2015年7月在南京軍區(qū)南京總醫(yī)院就診的初診2型糖尿病患者116例。采用電化學發(fā)光法測定其血清25(OH)D水平。根據(jù)不同水平將患者分為3組,即維生素D缺乏組:25(OH)D20ng/m L(n=34);維生素D不足組:20ng/m L≤25(OH)D30ng/m L(n=55);維生素D充足組:25(OH)D≥30 ng/m L(n=27)。所有患者均行饅頭餐-胰島素-C肽釋放試驗,比較3組患者的空腹血糖、空腹胰島素、空腹C肽、空腹胰高血糖素、穩(wěn)態(tài)模型胰島素抵抗指數(shù)(HOMA-IR)、胰島β細胞功能指數(shù)(HOMA-β)、早期相胰島素分泌指數(shù)(△I_(30)/△G_(30))、饅頭餐-胰島素-C肽釋放試驗30 min內(nèi)C肽曲線下面積(AUC_(CP30))、180 min內(nèi)C肽曲線下面積(AUC_(CP180))、葡萄糖曲線下面積(AUC_(PG))和胰高血糖素曲線下面積(AUC_(胰高血糖素))。結果 13組患者空腹血糖、空腹胰島素、空腹C肽、空腹胰高血糖素、HOMA-IR、HOMA-β、AUC_(胰高血糖素)比較,差異均無統(tǒng)計學意義(P0.05)。23組患者△I_(30)/△G_(30)、AUC_(CP30)、AUC_(CP180)、AUC_(PG)比較,差異均有統(tǒng)計學意義(P0.05);其中缺乏組患者△I_(30)/△G_(30)、AUC_(CP30)、AUC_(CP180)均低于不足組、充足組,而AUC_(PG)(52.57±7.45)則高于不足組(48.18±10.32)、充足組(44.39±9.05),差異有統(tǒng)計學意義(P0.05);不足組患者△I_(30)/△G_(30)、AUC_(CP30)、AUC_(CP180)均低于充足組[(3.07±1.60)vs(4.03±1.81)、(2.11±1.22)vs(3.14±1.93)、(15.18±5.24)vs(20.55±12.97),P0.05],而AUC_(PG)則高于充足組(48.18±10.32 vs 44.39±9.05,P0.05)。3Pearson相關分析結果顯示,25(OH)D與△I_(30)/△G_(30)、AUC_(CP30)、AUC_(CP180)呈正相關(r值分別為0.4、0.268、0.255,P0.01),與AUC_(PG)呈負相關(r=-0.403,P0.05);偏相關分析結果顯示,進一步校正年齡、性別、BMI、收縮壓、舒張壓、總膽固醇、三酰甘油、低密度脂蛋白膽固醇、高密度脂蛋白膽固醇后,25(OH)D依然與△I_(30)/△G_(30)、AUC_(CP30)、AUC_(CP180)正相關(r值分別為0.416、0.292、0.274,P0.01);與AUC_(PG)呈顯著負相關(r=-0.39,P0.01);Pearson相關分析及偏相關分析結果均顯示25(OH)D與HOMA-IR、HOMA-β、空腹胰高血糖素、AUC_(胰高血糖素)無顯著相關性(P0.05)。結論初診2型糖尿病患者25-羥維生素D缺乏與早期相胰島素分泌和總體胰島素分泌下降有關,與基礎胰島素分泌無關。25-羥維生素D水平與2型糖尿病患者胰島α細胞功能無明顯相關性。
