【摘要】：目的比較超聲引導(dǎo)下~(32)P磷酸鉻膠體(簡稱~(32)P)膠體、無水酒精及單純抽液介入治療甲狀腺囊性結(jié)節(jié)的臨床效果,探討最佳介入治療方法。方法將189例甲狀腺囊性結(jié)節(jié)患者隨機(jī)分為~(32)P膠體組71例、無水酒精組65例、單純抽液組53例。三組均于超聲引導(dǎo)下抽盡囊液,~(32)P膠體組注入~(32)P膠體1.85~3.70 MBq/cm3與地塞米松1~5 mg的混合液;無水酒精組按抽液量的1/3注入無水酒精;單純抽液組不注入藥物。比較三組的臨床療效、不良反應(yīng)發(fā)生率及二次治療率。結(jié)果~(32)P膠體組、無水酒精組、單純抽液組首次治療有效率分別為87.32%(62/71)、70.77%(46/65)、22.64%(12/53)。~(32)P膠體組首次治療有效率高于無水酒精組和單純抽液組(P0.05或0.01),且無水酒精組高于單純抽液組(P0.01)。~(32)P膠體組、無水酒精組、單純抽液組不良反應(yīng)發(fā)生率分別為4.2%(3/71)、10.8%(7/65)、3.8%(2/53)。三組不良反應(yīng)發(fā)生率比較,P均0.05。~(32)P膠體組、無水酒精組、單純抽液組分別有29例(40.8%)、46例(70.8%)、51例(96.2%),需進(jìn)行二次治療,其二次治療率依次升高(P均0.01)。結(jié)論超聲引導(dǎo)下~(32)P膠體介入治療甲狀腺囊性結(jié)節(jié)臨床效果較好、安全性較高。
[Abstract]:Objective to compare the clinical effects of ultrasound guided ~ (32) P chromic phosphate colloid (32) P) colloid), anhydrous alcohol and simple extraction in the treatment of thyroid cystic nodules, and to explore the best interventional therapy. Methods 189 patients with thyroid cystic nodules were randomly divided into ~ (32) P colloid group (n = 71), anhydrous alcohol group (n = 65) and simple extraction group (n = 53). All the three groups were injected with ~ (32) P colloidal solution (1.85 ~ 3.70 MBq/cm3) and dexamethasone (1 ~ 5 mg) under the guidance of ultrasound, the anhydrous alcohol group was injected with anhydrous alcohol according to 1 / 3 of the extracted volume, and the pure extract group was not injected with drugs. The clinical efficacy, adverse reaction rate and secondary treatment rate were compared among the three groups. Results the effective rates of the first treatment were 87.32% (62 / 71), 70.77% (46 / 65), 22.64% (12 / 53) .32 P colloidal group were higher than those in the anhydrous alcohol group and the pure extraction group (P0.05 or 0.01), and the effective rate of the absolute alcohol group was higher than that of the pure extract group (P0.01) .32 P colloid group. In anhydrous alcohol group, the incidence of adverse reactions was 4.2% (3 / 71) or 10.8% (7 / 65) or 3.8% (2 / 53) respectively. There were 29 cases (40.8%) and 46 cases (70.8%) with 51 cases (96.2%) in the three groups, respectively. The rate of secondary treatment was higher than that of the control group (P 0.01), and the incidence of adverse reactions was 0.05.32 P colloidal group, anhydrous alcohol group and pure liquid extraction group respectively (29 cases, 40.8%, 70.8%, 96.2%), and the rate of secondary treatment was higher than that of the control group (P 0.01). Conclusion Ultrasound-guided ~ (32) P colloid interventional therapy for thyroid cystic nodules is effective and safe.
