骨形態(tài)發(fā)生蛋白-7基因變異與太原地區(qū)漢族人群糖尿病及胰島素抵抗關(guān)聯(lián)研究

【摘要】:目的:探討骨形態(tài)發(fā)生蛋白-7(bone morphogenetic protein 7,BMP7)基因變異與太原地區(qū)漢族人群2型糖尿病、胰島素抵抗的關(guān)系,從分子遺傳學(xué)的角度和水平闡明太原地區(qū)漢族人群易患糖尿病的遺傳機制,為太原地區(qū)漢族人群2型糖尿病的防治提供一定的遺傳學(xué)證據(jù)。方法:選用以流行病學(xué)調(diào)查為基礎(chǔ)的病例-對照研究,選取173名(男性104人、女性69人)太原地區(qū)漢族人作為研究對象,按照1999年WHO制訂的2型糖尿病診斷標(biāo)準(zhǔn),將研究對象分成糖尿病組(90例,男性56例、女性34例)和正常對照組(83人,男性48人、女性35人)。選取2個具有代表性的BMP7基因位點在研究對象中進行SNa Pshot-PCR基因型鑒定,通過己糖激酶法測定空腹血糖(FBG,單位mmol/L),化學(xué)發(fā)光法測定空腹血胰島素(Fl NS,單位m IU/L),穩(wěn)態(tài)模型評估法(HOMA)計算胰島素抵抗指數(shù)(HOMA-IR=FBG×FINS/22.5),β細胞功能指數(shù)(HOMA-β)通過HOMA-β=20×FINS/(FBG-3.5)評估,并分析BMP7基因變異與2型糖尿病及胰島素抵抗的關(guān)聯(lián)性。結(jié)果:1.經(jīng)過Hardy-Weinberg檢驗可知:BMP7基因的rs6025422位點和rs17480735位點符合平衡定律(P0.05)。經(jīng)過卡方檢驗可知rs6025422基因位點分布在正常對照組和糖尿病組有統(tǒng)計學(xué)差異(P0.05),rs17480735基因位點無統(tǒng)計學(xué)差異(P0.05)。2.Logistic回歸分析結(jié)果顯示:rs6025422基因位點中GG基因型為糖尿病的保護因素,年齡、BMI為糖尿病的危險因素;3.糖尿病表型資料在基因rs6025422不同基因型組間的比較顯示:男性人群中不同基因型組間FBG、FINS、HOMA-IR有統(tǒng)計學(xué)差異(P0.05=,HOMA-β無統(tǒng)計學(xué)差異(P0.05)。FBG和HOMA-IR在校正P值后差異仍有統(tǒng)計學(xué)意義(Pα0.05),經(jīng)比較可知FBG、FINS、HOMA-IR在基因AA、AG、GG間呈趨勢性降低;總?cè)巳褐蠪INS和HOMA-IR有統(tǒng)計學(xué)差異,HOMA-β無統(tǒng)計學(xué)差異(P0.05);女性人群中各指標(biāo)在基因型之間無統(tǒng)計學(xué)差異(P0.05)。結(jié)論:1.BMP7基因的rs6025422基因位點中GG基因型為糖尿病的保護因素,年齡、BMI為糖尿病的危險因素;2.糖尿病表型資料在基因rs6025422不同基因型組間的比較顯示男性人群中不同基因型組間FBG、FINS、HOMA-IR有統(tǒng)計學(xué)差異(P0.05),HOMA-β無統(tǒng)計學(xué)差異(P0.05)。FBG和HOMA-IR在校正P值后差異仍有統(tǒng)計學(xué)意義(Pα0.05),經(jīng)比較可知FBG、FINS、HOMA-IR在基因AA、AG、GG間呈趨勢性降低;總?cè)巳褐蠪INS和HOMA-IR有統(tǒng)計學(xué)差異,HOMA-β無統(tǒng)計學(xué)差異(P0.05);女性人群中各指標(biāo)在基因型之間無統(tǒng)計學(xué)差異(P0.05)。3.BMP7基因變異可能與太原地區(qū)漢族人群2型糖尿病與胰島素抵抗相關(guān)。
[Abstract]:Objective: to investigate the relationship between the variation of bone morphogenetic protein-7 (bone morphogenetic protein 7 (BMP7) gene and type 2 diabetes mellitus and insulin resistance in Taiyuan Han population, and to elucidate the genetic mechanism of the susceptibility to diabetes in Taiyuan Han population from the perspective of molecular genetics. To provide some genetic evidence for the prevention and treatment of type 2 diabetes in Taiyuan Han population. Methods: a case-control study based on epidemiological investigation was conducted. 173 Han Chinese (104 males and 69 females) in Taiyuan were selected as the subjects. The diagnostic criteria for type 2 diabetes were established by WHO in 1999. The subjects were divided into diabetic group (90 cases, male 56 cases, female 34 cases) and normal control group (83 cases, male 48, female 35). Two representative BMP7 loci were selected to identify SNa Pshot-PCR genotypes. Fasting blood glucose (FBG, unit mmol/L) was measured by hexokinase assay, fasting insulin (Fl NS, unit m IU/L was measured by chemiluminescence assay, insulin resistance index (HOMA-IR=FBG 脳 FINS/22.5) was calculated by steady-state model evaluation method (HOMA), and 尾 cell function index (HOMA- 尾) was evaluated by HOMA- 尾 -20 脳 FINS/ (FBG-3.5). The association of BMP7 gene mutation with type 2 diabetes mellitus and insulin resistance was analyzed. The result is 1: 1. Hardy-Weinberg test showed that the rs6025422 and rs17480735 loci of the BMP7 gene obeyed the equilibrium law (P0.05). After chi-square test, the distribution of rs6025422 gene loci in normal control group and diabetes group was statistically different (P0.05). There was no significant difference in rs17480735 gene locus (P0.05). 2. Logistic regression analysis showed that GG genotype in the GG gene locus was the protective factor of diabetes mellitus. Age BMI is a risk factor for diabetes. The comparison of phenotypic data of diabetes mellitus among different genotypes of gene rs6025422 showed that there were significant differences in FBG,FINS,HOMA-IR between different genotypes of male population (P0.05Homa 尾) (P0.05) .FBGs and HOMA-IR were still significantly different after correction of P value (P 偽 0.05). The results showed that FBG,FINS,HOMA-IR decreased gradually between gene AA,AG,GG. There was no statistical difference between FINS and HOMA-IR in the total population (P0.05), and there was no statistical difference among the genotypes in the female population (P0.05).
