【摘要】：一、絕經(jīng)后骨質(zhì)疏松癥干預(yù)概況隨著絕經(jīng)后骨質(zhì)疏松癥發(fā)病率的逐年增加,絕經(jīng)后骨質(zhì)疏松癥的防治越來越受到重視。大量臨床試驗(yàn)對(duì)絕經(jīng)后骨質(zhì)疏松癥干預(yù)的方案進(jìn)行了研究,并探討了各方案的有效性和安全性。生活方式、西藥、中醫(yī)藥及相互聯(lián)用的干預(yù)方式對(duì)絕經(jīng)后骨質(zhì)疏松癥的防治均具有臨床意義,然而目前臨床面臨的問題是,在眾多的方案中如何針對(duì)不同患者進(jìn)行個(gè)體化的干預(yù)措施的選擇。通過查閱大量文獻(xiàn),筆者對(duì)已用于臨床的干預(yù)方式、方法進(jìn)行了文獻(xiàn)研究,按照基礎(chǔ)治療、西藥治療、中醫(yī)中藥治療的提綱對(duì)其干預(yù)研究及療效進(jìn)行了綜述。通過綜述進(jìn)一步肯定了上述方案的有效性,并為下一步比較分析各方案有效性、安全性提供了初步的研究思路和文獻(xiàn)證據(jù)。二、中藥干預(yù)絕經(jīng)后骨質(zhì)疏松的系統(tǒng)評(píng)價(jià)目的：以對(duì)照組作為對(duì)照,評(píng)價(jià)中藥干預(yù)絕經(jīng)后骨質(zhì)疏松癥的療效性和安全性。方法：按照Cochrane協(xié)作網(wǎng)的系統(tǒng)評(píng)價(jià)要求,電子檢索Cochrane Library、 PubMed、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫、清華同方CNKI數(shù)據(jù)庫、維普中文科技期刊數(shù)據(jù)庫、萬方學(xué)位論文數(shù)據(jù)庫、萬方中華醫(yī)學(xué)會(huì)期刊數(shù)據(jù)庫,并手工檢索相關(guān)期刊雜志。采用Revman5.2軟件對(duì)篩選出的相關(guān)文獻(xiàn)數(shù)據(jù)資料進(jìn)行比較分析。結(jié)果：共納入14篇文獻(xiàn),包括1119例糖尿病前期患者。綜合分析顯示：(1)中藥與單純西藥比較,前者對(duì)中醫(yī)臨床證候的改善效果明顯；(2)兩者相比較,中藥增加骨密度水平、血清雌二醇濃度,改善骨代謝指標(biāo)、細(xì)胞因子水平的作用方面,表現(xiàn)出比使用單純西藥治療更加確實(shí)的療效,而且未發(fā)現(xiàn)明顯或者嚴(yán)重不良反應(yīng)；(3)兩者安全性作用效果相當(dāng)。結(jié)論：中藥能夠增加骨密度、血清雌二醇濃度,調(diào)節(jié)骨代謝,改善臨床癥狀和中醫(yī)證候,且其不良反應(yīng)發(fā)生率較低,對(duì)臨床絕經(jīng)后骨質(zhì)疏松癥的預(yù)防具有重要意義。
[Abstract]:1. With the increasing incidence of postmenopausal osteoporosis, the prevention and treatment of postmenopausal osteoporosis are paid more and more attention. A large number of clinical trials were carried out to study the intervention scheme of postmenopausal osteoporosis, and the effectiveness and safety of each protocol were discussed. Lifestyle, western medicine, Chinese medicine and their combined intervention are all of clinical significance for the prevention and treatment of postmenopausal osteoporosis. However, the current clinical problems are: How to choose individual interventions for different patients in a wide range of programs. Through consulting a large number of literature, the author has carried on the literature research to the intervention method which has already used in the clinic, has carried on the literature research, according to the basic treatment, the western medicine treatment, the traditional Chinese medicine treatment outline has carried on the summary to its intervention research and the curative effect. The effectiveness of these schemes is further confirmed through a review, and preliminary research ideas and documentary evidence are provided for the further comparison and analysis of the effectiveness and safety of each scheme. Objective: to evaluate the efficacy and safety of traditional Chinese medicine in the treatment of postmenopausal osteoporosis. Methods: according to the systematic evaluation requirements of Cochrane Cooperative Network, we searched electronically the Cochrane Library, PubMed, Chinese Biomedical Literature Database, Tsinghua Tongfang CNKI Database, Weipu Chinese Science and Technology Journal Database and Wanfang dissertations Database. Wanfang Chinese Medical Association journal database, and manual retrieval of related journals. Using Revman 5.2 software, the selected literature data were compared and analyzed. Results: a total of 14 articles were included, including 1119 prediabetic patients. Comprehensive analysis shows that: (1) compared with western medicine, the former has obvious effect on the improvement of TCM clinical syndromes; (2) compared with traditional Chinese medicine, traditional Chinese medicine increases bone mineral density, serum estradiol concentration, and improves bone metabolism index. The effect of cytokine level was more effective than that of western medicine alone, and no obvious or serious adverse reactions were found. (3) the effect of safety was similar. Conclusion: traditional Chinese medicine can increase bone mineral density, serum estradiol concentration, regulate bone metabolism, improve clinical symptoms and TCM syndromes, and the incidence of adverse reactions is relatively low, which is of great significance for the prevention of postmenopausal osteoporosis.
