【摘要】：目的探討炎性因子水平與糖尿病腎病(DN)分期的關系。方法選取2014年5月—2016年5月江蘇省宜興市人民醫(yī)院腎內(nèi)科收治的2型糖尿病患者70例,根據(jù)尿微量清蛋白排泄率分為正常蛋白尿組(25例)、微量清蛋白尿組(22例)和大量蛋白尿組(23例),另選取同期在本院進行健康體檢的自愿者25例作為對照組。采用酶聯(lián)免疫吸附試驗檢測各組患者尿液白介素18(IL-18)和單核細胞趨化蛋白1(MCP-1)水平進行比較。結果 4組年齡、體質指數(shù)(BMI),舒張壓比較,差異無統(tǒng)計學意義(P0.05);4組病程、收縮壓、尿微量清蛋白比較,差異有統(tǒng)計學意義(P0.05);4組IL-18和MCP-1水平比較,差異有統(tǒng)計學意義(P0.05);4組空腹血糖(FPG)、糖化血紅蛋白(HbA1c)、三酰甘油(TG)、低密度脂蛋白膽固醇(LDL-C)、超敏反應蛋白(hs-CRP)、白介素6(IL-6)、腫瘤壞死因子α(TNF-α)、尿素氮(BUN)及血肌酐(Scr)水平比較,差異有統(tǒng)計學意義(P0.05,)。結論炎性因子水平隨著DN病情加重而升高,檢測炎性因子水平可作為判斷病情程度的輔助指標。
[Abstract]:Objective to investigate the relationship between the level of inflammatory factors and the stage of diabetic nephropathy (DN). Methods from May 2014 to May 2016, 70 patients with type 2 diabetes mellitus were selected from Department of Renal Medicine, Yixing City people's Hospital, Jiangsu Province. According to the urinary microalbumin excretion rate, the patients were divided into normal albuminuria group (n = 25), microalbuminuria group (n = 22) and large albuminuria group (n = 23). The levels of interleukin-18 (IL-18) and monocyte chemoattractant protein-1 (MCP-1) in urine were measured by enzyme-linked immunosorbent assay (Elisa). Results the age, body mass index (BMI), diastolic blood pressure (DBP) of the four groups were not significantly different (P0.05). There were significant differences in the levels of IL-18 and MCP-1 between the four groups (P0.05), including the course of disease, systolic blood pressure (SBP) and urine microalbumin (P 0.05). There were significant differences in fasting blood glucose (FPG), glycosylated hemoglobin (HbA1c), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), hypersensitive protein (hs-CRP), interleukin-6 (IL-6), tumor necrosis factor- 偽 (TNF- 偽), urea nitrogen (bun) and serum creatinine (SCR). The difference was statistically significant (P0.05,). Conclusion the level of inflammatory factors increases with the severity of DN, and the level of inflammatory factors can be used as an auxiliary index to judge the severity of DN.
