【摘要】：目的:通過分析2型糖尿病(type 2 diabetes mellitus,T2DM)患者晚期糖基化終產(chǎn)物(advanced glycation end products,AGEs)水平與腎功能的關(guān)系,為糖尿病腎臟病變(diabetic kidney disease,DKD)的預(yù)防及早期診斷提供依據(jù)。方法:收集2016.07.01-2017.01.31在山東省立醫(yī)院內(nèi)分泌科住院治療的T2DM患者共532人,根據(jù)納入排除標(biāo)準(zhǔn),最終共有189例患者進入研究當(dāng)中,利用糖基化終產(chǎn)物無創(chuàng)檢測儀測定皮膚AGEs的水平,并收集血清肌酐、估計腎小球濾過率(estimated glomerular filtration rate,eGFR)、尿白蛋白肌酐比(urinary albumin/creatinine ratio,UACR)及胱抑素C等指標(biāo)。以皮膚AGEs的四分位數(shù)為界點,分別以AGEs水平≤73.3、73.3-81.3、81.3-93.9、≥93.9,將人群分為四組。利用spss17.0軟件進行統(tǒng)計學(xué)分析,應(yīng)用單因素方差分析及非參數(shù)檢驗比較組間的差異,并應(yīng)用多重線性回歸分別分析AGEs與肌酐、eGFR、UACR和胱抑素C的相關(guān)性。結(jié)果:1.本次研究共納入T2DM患者189例,其中男性101例,女性88例,平均年齡60±11.986歲,糖尿病病程10(11)年,平均皮膚AGEs水平84±14.68。2.四組之間eGFR、胱抑素C差別存在統(tǒng)計學(xué)差異,分別為eGFR(107.68 vs.98.61、93.09、89.55,P值分別為0.031、0.000和0.000,均小于0.05),胱抑素C(0.85vs.0.92、1.01、1.13,P值分別為0.007、0.000和0.000,均小于0.05)。四組間log肌酐和logUACR雖然差異沒有統(tǒng)計學(xué)意義,但隨著AGEs升高,兩者有逐漸升高的趨勢。3.多重線性回歸分析顯示AGEs水平與eGFR呈負相關(guān)關(guān)系(beta=-0.252,P=0.001);與胱抑素C呈正相關(guān)關(guān)系(beta=0.236,P=0.01)。log肌酐與AGEs在單因素線性回歸分析時呈正相關(guān)關(guān)系,而在校正了其他因素之后兩者相關(guān)性無統(tǒng)計學(xué)意義;logUACR在單因素回歸分析時與AGEs即無統(tǒng)計學(xué)意義,未進一步行多重線性回歸分析。結(jié)論:AGEs水平與2型糖尿病患者腎功能相關(guān),其水平可以反映腎功能的情況。
[Abstract]:Objective: to analyze the relationship between advanced glycation end product (advanced glycation end products ages and renal function in patients with type 2 diabetes mellitus (type 2 diabetes mellitusus T2DM), so as to provide evidence for the prevention and early diagnosis of (diabetic kidney disease of diabetic nephropathy. Methods: a total of 532 T2DM patients were collected from January 31, 2016.07.01-2017.01.31 in Department of Endocrinology, Shandong Provincial Hospital. According to the exclusion criteria, 189 patients were included in the study. Serum creatinine was collected to estimate glomerular filtration rate (estimated glomerular filtration), urinary albumin creatinine ratio (urinary albumin/creatinine / UACR) and cystatin C.
