裸燕麥對(duì)2型糖尿病合并高血壓患者血壓的影響研究

【摘要】:背景醫(yī)學(xué)營(yíng)養(yǎng)療法對(duì)2型糖尿病合并高血壓患者的疾病管理具有重要作用,但具體營(yíng)養(yǎng)干預(yù)措施尚不統(tǒng)一。目的在結(jié)構(gòu)化膳食控制結(jié)合健康教育的基礎(chǔ)上,探討全胚芽裸燕麥(NOG)對(duì)2型糖尿病合并高血壓患者血壓的影響。方法采用自身前后對(duì)照和多組平行對(duì)照的方法,以電話、廣告途徑在全國范圍內(nèi)招募符合納入標(biāo)準(zhǔn)的受試者59例。采用隨機(jī)數(shù)字表法將其分為A組(20例)、B組(n=20)和C組(n=19),其中A組給予結(jié)構(gòu)化膳食控制及健康教育干預(yù),B組和C組在A組的基礎(chǔ)上分別給予50 g、100 g NOG干預(yù),于每日早、晚兩餐采用NOG粥代替能量相同的其他主食,干預(yù)28 d。干預(yù)前采集基線指標(biāo),并根據(jù)WHO標(biāo)準(zhǔn),將所有受試者分為正常體質(zhì)量〔18.5kg/m2體質(zhì)指數(shù)(BMI)25.0 kg/m2〕、超重(25.0 kg/m2≤BMI30.0 kg/m2)、肥胖(BMI≥30.0 kg/m2)。于干預(yù)前及干預(yù)第28天測(cè)量所有受試者的收縮壓(SBP)及舒張壓(DBP)。結(jié)果 3組受試者性別、年齡、病程、空腹血糖(FBG)、餐后2 h血糖(PBG)、糖化血紅蛋白、BMI比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。自身前后對(duì)照:3組受試者干預(yù)第28天SBP、DBP均分別低于同組干預(yù)前,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。組間比較:3組受試者干預(yù)前后SBP變化值比較,差異有統(tǒng)計(jì)學(xué)意義(F=4.12,P=0.02);DBP變化值比較,差異無統(tǒng)計(jì)學(xué)意義(F=0.24,P=0.79)。雙因素方差分析:3組受試者SBP干預(yù)前后變化值組間與BMI間無交互作用(P0.05),干預(yù)措施對(duì)SBP的影響有統(tǒng)計(jì)學(xué)意義(P0.05),BMI對(duì)SBP的影響無統(tǒng)計(jì)學(xué)意義(P0.05);3組受試者DBP干預(yù)前后變化值組間與BMI間無交互作用(P0.05),干預(yù)措施對(duì)DBP的影響無統(tǒng)計(jì)學(xué)意義(P0.05),BMI對(duì)DBP的影響無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論裸燕麥攝入及結(jié)構(gòu)化膳食控制對(duì)2型糖尿病合并高血壓患者均具有降血壓效果,兩者結(jié)合優(yōu)于單純的結(jié)構(gòu)化膳食控制,且對(duì)SBP降低效果更明顯。裸燕麥的降血壓效果不受BMI的影響。
[Abstract]:Background Medical nutrition therapy plays an important role in disease management of patients with type 2 diabetes mellitus and hypertension, but the specific nutritional intervention measures are not uniform. Objective to investigate the effects of structured diet control and health education on blood pressure in patients with type 2 diabetes mellitus complicated with hypertension. Methods 59 subjects who met the inclusion criteria were recruited by telephone and advertisement methods. They were randomly divided into two groups: group A (n = 20) and group C (n = 19). Group A was treated with structured diet control and health education intervention. Group B and C were given 50g / 100g NOG on the basis of group A, respectively. Two late meals were treated with NOG porridge instead of other main foods with the same energy.
