【摘要】：目的探討克羅米芬聯(lián)合地塞米松對多囊卵巢綜合征(PCOS)的治療效果。方法選取PCOS患者120例,隨機分為觀察組和對照組各60例。兩組均于月經(jīng)后第5天連續(xù)5天口服克羅米芬(50 mg/d),觀察組月經(jīng)周期第1天口服地塞米松(1片/次,1次/d),連續(xù)15天;兩組均治療3個月。統(tǒng)計治療3個月內兩組不良反應發(fā)生情況及排卵率、妊娠率;分別于治療前后檢測血清卵泡刺激素(FSH)、促黃體生成素(LH)、睪酮(T)、雌二醇(E2)、胰島素(INS),卵泡液IL-6、TNF-α、IL-1β及總抗氧化能力(TAC)和超氧化物歧化酶(SOD)、過氧化氫酶(CAT)、谷胱甘肽過氧化物酶(GPx)、谷氨酸半胱氨酸連接酶(Gcl-c)。結果觀察組排卵率及妊娠率均高于對照組(P均0.05),兩組不良反應發(fā)生率比較差異無統(tǒng)計學意義(P0.05)。兩組治療后血清FSH、LH、T、E2、INS水平均較治療前降低(P均0.05)。治療后兩組TAC、SOD、GPx均升高,卵泡液IL-6、TNF-α、IL-1β均降低,但觀察組變化更顯著(P均0.05)。兩組治療前后CAT、Gcl-c比較P均0.05。結論克羅米芬聯(lián)合地塞米松可促進PCOS患者排卵及妊娠,且效果好于單用克羅米芬。
[Abstract]:Objective to investigate the therapeutic effect of clomiphene combined with dexamethasone on polycystic ovary syndrome (PCOS). Methods 120 patients with PCOS were randomly divided into observation group (n = 60) and control group (n = 60). In both groups, clomiphene 50 mg / d was given orally for 5 days after menstruation, and dexamethasone was given orally for 15 days on the first day of menstrual cycle in the observation group, and the two groups were treated for 3 months. The incidence of adverse reactions, ovulation rate and pregnancy rate in the two groups were statistically analyzed within 3 months. Serum follicle stimulating hormone (FSH), luteinizing hormone (LHN), testosterone (TX), estradiol (E2P), insulin (INSN), IL-6TNF- 偽 (IL-1 尾), superoxide dismutase (SOD), catalase (CAT), glutathione peroxides (GSH) were detected before and after treatment. Glutamic acid cysteine ligase Gcl-c. Results the ovulation rate and pregnancy rate in the observation group were higher than those in the control group (P < 0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P 0.05). After treatment, the serum levels of FSHLHN, THX, E2 and ins in both groups were lower than those before treatment (P < 0.05). After treatment, the levels of tact SOD GPX in both groups were increased, and IL-6 TNF- 偽 IL-1 尾 was decreased in follicular fluid, but the changes in observation group were more significant than those in control group (P < 0.05). The comparison of Gcl-c between the two groups before and after treatment (P < 0.05). Conclusion clomiphene combined with dexamethasone can promote ovulation and pregnancy in patients with PCOS, and the effect is better than that of clomiphene alone.
