【摘要】：目的探討糖尿病足部潰瘍感染病原菌的特點以及相關危險因素,并針對相關因素提出預防及干預措施,以促進患者早日康復。方法回顧性分析2012年2月至2014年5月治療的120例糖尿病足患者臨床資料,將發(fā)生糖尿病足部潰瘍感染的38例患者作為感染組,未發(fā)生潰瘍感染的82例患者作為非感染組,分析糖尿病足部潰瘍感染的病原菌特點和影響感染相關危險因素。結果該研究38例感染患者中共分離出病原菌43株,革蘭陽性菌22株,占51.2%;革蘭陰性菌18株,占41.9%;真菌3株,占7.0%。位居前3的病原菌為金黃色葡萄球菌、銅綠假單胞菌和陰溝腸桿菌,均為7株,各占16.3%。單因素分析結果顯示,合并糖尿病血管并發(fā)癥、合并視網膜病變、潰瘍數(shù)目2個、合并骨髓炎以及合并腎病等與感染密切相關(P0.05);多因素分析結果顯示,高密度脂蛋白膽固醇、血紅蛋白、糖尿病血管并發(fā)癥以及潰瘍數(shù)目、骨髓炎是糖尿病足部潰瘍感染的獨立危險因素。結論糖尿病足部潰瘍感染是多種因素共同作用而致,因此臨床應加強對這些因素的預防和控制,以降低糖尿病足部潰瘍的感染率。
[Abstract]:Objective to investigate the characteristics of pathogenic bacteria of diabetic foot ulcer infection and the related risk factors, and to put forward prevention and intervention measures to promote the early recovery of patients with diabetic foot ulcers. Methods the clinical data of 120 diabetic foot patients treated from February 2012 to May 2014 were retrospectively analyzed. 38 patients with diabetic foot ulcer infection and 82 patients without ulcer infection were selected as infection group. To analyze the pathogenic bacteria characteristics of diabetic foot ulcer infection and the risk factors related to infection. Results A total of 43 pathogenic bacteria were isolated from 38 infected patients, 22 Gram-positive bacteria (51.2%), 18 Gram-negative bacteria (41.9%) and 3 fungi (7.0%). The first 3 pathogens were Staphylococcus aureus, Pseudomonas aeruginosa and Enterobacter cloacae. Univariate analysis showed that the number of diabetic vascular complications, retinopathy, ulcers, osteomyelitis and nephropathy were closely related to infection (P 0.05). Hemoglobin, diabetic vascular complications and number of ulcers, osteomyelitis are independent risk factors for diabetic foot ulcer infection. Conclusion the infection of diabetic foot ulcers is caused by many factors, so the prevention and control of these factors should be strengthened in order to reduce the infection rate of diabetic foot ulcers.
