【摘要】：目的探討椎體成形術(shù)(PVP)和椎體后凸成形術(shù)(PKP)治療老年骨質(zhì)疏松性椎體壓縮性骨折的臨床療效。方法采用PVP和PKP治療老年骨質(zhì)疏松性椎體壓縮性骨折患者72例,其中PVP組40例,PKP組32例。比較兩組手術(shù)時間、骨水泥注入量、骨水泥滲漏率、椎體恢復(fù)高度、住院天數(shù)、手術(shù)前后視覺模擬評分(VAS)。結(jié)果兩組手術(shù)時間、椎體恢復(fù)高度、骨水泥滲漏、骨水泥注入量比較差異有統(tǒng)計學(xué)意義(P0.05)。兩組住院天數(shù)、手術(shù)前后VAS比較差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論 PVP和PKP對于老年骨質(zhì)疏松性椎體壓縮性骨折均具有快速、有效緩解疼痛的作用。PVP操作簡便,手術(shù)時間短,但滲漏率高,無法恢復(fù)椎體高度。PKP有利于恢復(fù)椎體高度,減少滲漏,但操作復(fù)雜,手術(shù)時間長,過度恢復(fù)椎體高度可增加椎體再骨折的風險。
[Abstract]:Objective to investigate the clinical effect of vertebroplasty and kyphoplasty (PKP) in the treatment of senile osteoporotic vertebral compression fracture. Methods 72 cases of senile osteoporotic vertebral compression fracture were treated with PVP and PKP, including 40 cases of PVP group and 32 cases of PVP group. The time of operation, the amount of bone cement injected, the leakage rate of bone cement, the height of vertebral body recovery, the days of hospitalization, the visual analogue score before and after operation were compared between the two groups. Results the time of operation, the height of vertebral body recovery, the leakage of bone cement and the amount of bone cement injection were significantly different between the two groups (P 0.05). There was no significant difference in VAS between the two groups before and after operation (P 0.05). Conclusion both PVP and PKP have a rapid and effective analgesic effect on osteoporotic vertebral compression fracture. PVP is easy to operate, short operation time, but high leakage rate, which can not restore vertebral height .PKP is beneficial to restore vertebral height. Reduce leakage, but complicated operation, long operation time, excessive recovery of vertebral body height can increase the risk of vertebral refracture.
