廣西柳江縣成人肥胖測量指標與血壓、血糖、血脂的相關因素研究

【摘要】:目的:探討廣西柳江縣成人中以體重指數(shù)(BMI)、腰圍(WC)、內臟脂肪指數(shù)(VAI)為代表的肥胖指標與血壓、血糖、血脂的關系。為預防和控制高血壓、糖尿病、高血脂等肥胖相關慢性疾病的發(fā)生提供參考依據(jù)。方法:2013年12月至2014年1月,對柳江縣人群進行流行病學調查681人,實際有效數(shù)值399例。記錄一般測量指標,包括身高、體重、腰圍、內臟脂肪率、血壓,并進一步抽取血液標本送至實驗室查空腹血糖(FPG)、總膽固醇(TC)、甘油三酯(TG)、高密度脂蛋白膽固醇(HDL-C)、低密度脂蛋白膽固醇(LDL-C)。結果:①在廣西柳江縣399名受檢者中,男性較女性更易患肥胖癥、血壓異常、血脂異常。②分別依據(jù)BMI、WC、VAI數(shù)值進行分組后比較,血壓異常(高于正常范圍)、高血糖、血脂異常檢出率(患病率)的差異有統(tǒng)計學意義,超重及肥胖組中上述病癥的患病率高于非超重或肥胖組。③男性中,VAI與血壓、TG呈正相關,與HDL-C呈負性相關,具有統(tǒng)計學意義(p0.05);女性中,VAI與血壓、血脂均存在明顯相關關系(p0.05):VAI與MAP(平均動脈壓)、TG、TC、LDL-C呈正相關,VAI與HDL-C呈負相關,具有統(tǒng)計學意義(p0.05)。女性人群中年齡與FPG (r=0.253, p=0.000)、年齡與VAI (r=0.163, p=0.008)均有顯著相關性,具有統(tǒng)計學意義(p0.05)。在混雜有年齡因素時FPG與VAI相關性分析結果提示男女組中均有顯著相關性(r男=0.183 VS r女=0.141),具有統(tǒng)計學意義(p0.05)。而排除年齡因素后,男女組中FPG與VAI均無明顯相關性。④二項分類資料logistic回歸結果表明,當WC(男性)85cm,WC(女性)80cm時,血壓異常的患病風險OR=2.836,血脂異常的患病風險OR=1.736,高血糖的患病風險是OR=2.835。當BMI≥23 kg/m2時,血壓異常的患病風險OR=3.541,血脂異常的患病風險OR=2.037,高血糖的患病風險OR=1.862(p0.05)。當BMI、WC數(shù)值均達到超重或肥胖的標準時,血壓異常、血脂異常、高血糖的患病風險OR值均大于單一項目達標時的OR值。結論:①在廣西柳江縣399名受檢者中,男性較女性更易患肥胖癥、血壓異常、血脂異常。②隨著BMI、WC、VAI的增加,血壓異常、高血糖、血脂異常的檢出率呈上升趨勢。③女性人群中年齡與FPG及VAI存在相關關系,VAI與血脂的相關性較男性明顯。④BMI是血壓異常(高于正常)、血脂異常的重要預測因子,WC是高血糖的重要預測因子。BMI、WC對血壓異常(高于正常)、血脂異常、高血糖的患病情況起著協(xié)同作用。
[Abstract]:Objective: to investigate the relationship between body mass index (BMI), waist circumference (WCC), visceral fat index (VAII) and blood pressure, blood glucose and blood lipid in adults in Liujiang County, Guangxi. To prevent and control hypertension, diabetes, hyperlipidemia and other obesity related chronic diseases. Methods: from December 2013 to January 2014, 681 people were investigated in Liujiang County. Record general measurements, including height, weight, waist circumference, visceral fat rate, blood pressure, Blood samples were collected and sent to the laboratory for fasting blood glucose (FPG), total cholesterol (TC), triglyceride (TGN), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C). Results among 399 subjects in Liujiang County, Guangxi, males were more prone to obesity, abnormal blood pressure and abnormal blood lipids. The prevalence of these disorders in overweight and obesity group was higher than that in non-overweight or obese group. There was a positive correlation between blood pressure and TG, and a negative correlation between VAI and HDL-C. There was a significant correlation between VAI and blood pressure and blood lipid in female patients (P 0.05). There was a positive correlation between VAI and MAPP (mean arterial pressure) TGG and TCLDL-C were negatively correlated with HDL-C, and there was significant difference between them (p 0.05). There was a significant correlation between age and FPG 0.253, p0. 000, VAI 0.163, p0. 008) in female population, which had statistical significance (P 0. 05). The results of correlation analysis between FPG and VAI showed that there was a significant correlation between FPG and VAI in male and female groups (0.183 vs 0.141), which had statistical significance (p 0.05). After the age factor was excluded, there was no significant correlation between FPG and VAI in male and female groups. 4.
