【摘要】：目的了解德宏州盈江縣暗娼人群中艾滋病病毒(HIV)感染率及其影響因素。方法采用橫斷面調(diào)查方法,于2011-2015年間,對盈江縣娛樂場所暗娼進(jìn)行問卷調(diào)查,收集人口學(xué)特征、高危行為、吸毒史、性病發(fā)生史等相關(guān)信息;并采集血樣進(jìn)行HIV抗體檢測。采用Logistic回歸模型分析HIV感染的危險(xiǎn)因素。結(jié)果 2011-2015年盈江縣共調(diào)查暗娼2 593人,全部納入研究,共發(fā)現(xiàn)HIV抗體陽性35例,其中外籍占60.0%(21/35),平均HIV感染率為1.3%。2011-2015年,暗娼人群各年的HIV感染率分別為2.8%、2.5%、1.4%、0.2%和0.8%。Logistic多因素回歸分析顯示:戶籍、民族、婚姻狀態(tài)、工作場所檔次、此次此地工作時間、最近一個月與客人發(fā)生性行為時安全套使用頻率,與暗娼人群的HIV感染率有統(tǒng)計(jì)學(xué)關(guān)聯(lián);離異或喪偶者[比值比(OR)=6.15,95%可信區(qū)間(CI):2.11~17.91]、外籍者(OR=7.06,95%CI:2.89~17.23)及未堅(jiān)持使用安全套者(OR=7.52,95%CI:2.14~26.37)感染HIV風(fēng)險(xiǎn)高。結(jié)論盈江縣暗娼人群HIV感染率平均水平較低,但外籍者、中低檔暗娼感染率高,且低檔暗娼逐年增加,中檔逐年減少。在以后的工作中,除繼續(xù)推廣100%使用安全套外,還應(yīng)重點(diǎn)加強(qiáng)低檔及跨境外籍暗娼的干預(yù)管理。
[Abstract]:Objective to investigate the HIV / AIDS infection rate and its influencing factors among prostitutes in Yingjiang County, Dehong. Methods A cross-sectional survey was carried out in Yingjiang County from 2011 to 2015 to collect demographic characteristics, high-risk behaviors, history of drug abuse, history of venereal diseases, and blood samples for detection of HIV antibodies. Logistic regression model was used to analyze the risk factors of HIV infection. Results A total of 2 593 prostitutes were investigated in Yingjiang County from 2011 to 2015, all of whom were included in the study. A total of 35 cases were found to be positive for HIV antibody, among which 60.021 / 35 were foreign sex workers, and the average HIV infection rate was 1.3%. 2011-2015. The HIV infection rate among prostitutes in each year was 2.8%, 2.5% and 1.4%, respectively, and 0.8%.Logistic regression analysis showed that: household registration, nationality, marital status, workplace grade, the working hours here and the frequency of condom use when having sex with guests in the last month. The rate of HIV infection in divorced or widowed women was significantly higher than that in those who divorced or lost their spouse [the ratio was 6.1595 95% CI: 2.111.17.91], the foreign population was 7.06% 2.8995 CI: 17.23) and those who did not insist on condom use had a higher risk of HIV infection than 7.52 95 CIW 2.144.37). Conclusion the average level of HIV infection rate among prostitutes in Yingjiang County is low, but the infection rate of middle and low class prostitutes is high, and the number of low class prostitutes increases year by year and decreases year by year. In the future, in addition to promoting 100% condom use, the intervention management of low-grade and cross-border foreign prostitutes should be emphasized.
