DWI在均質(zhì)甲狀腺微小結(jié)節(jié)性質(zhì)判斷中的應(yīng)用

【摘要】:目的探究磁共振彌散加權(quán)成像(DWI)在均質(zhì)甲狀腺微小結(jié)節(jié)性質(zhì)判斷中的價值。方法選取經(jīng)手術(shù)病理證實的162例均質(zhì)甲狀腺微小結(jié)節(jié)患者,共226個病灶進行磁共振常規(guī)掃描和DWI檢查。在226個均質(zhì)甲狀腺微小結(jié)節(jié)病灶的術(shù)后病理診斷中,良性結(jié)節(jié)47個,惡性結(jié)節(jié)179個。將所得全部病灶的影像學數(shù)據(jù)進行統(tǒng)計學分析。結(jié)果術(shù)后病理結(jié)果證實的47個良性結(jié)節(jié)中DWI呈高信號者13個,等信號者32個,低信號者2個;179個惡性結(jié)節(jié)中DWI呈高信號者135個,等信號者36個,低信號者8個。取b值為0 s/mm~2時,DWI掃描所得出的良性和惡性結(jié)節(jié)ADC值分別為(2.481±0.328)、(1.920±0.216)×10~(-3)mm~2/s;取b值為300 s/mm~2時,ADC值分別為(2.322±0.528)、(1.634±0.387)×10~(-3)mm~2/s;取b值為500 s/mm~2時,ADC值分別為(1.972±0.479)、(1.496±0.272)×10~(-3)mm~2/s;取b值為800 s/mm~2時,ADC值分別為(1.675±0.503)、(1.247±0.248)×10~(-3)mm~2/s;取b值為1 000 s/mm~2時,ADC值分別為(1.528±0.310)、(1.229±0.106)×10~(-3)mm~2/s;取b值為300、500和800 s/mm~2時,良性和惡性結(jié)節(jié)DWI掃描所得出的ADC值差異有統(tǒng)計學意義(P均0.05);而在b值為0s/mm~2和1 000 s/mm~2時,良性和惡性結(jié)節(jié)DWI掃描所得出的ADC值差異無統(tǒng)計學意義(P均0.05)。結(jié)論取b值為300、500和800 s/mm~2時,應(yīng)用DWI掃描所得出的ADC值可用于判斷均質(zhì)甲狀腺微小結(jié)節(jié)的病變性質(zhì)。
[Abstract]:Objective to investigate the value of diffusion-weighted magnetic resonance imaging (DWI) in the diagnosis of small homogeneous thyroid nodules. Methods A total of 226 lesions were examined by conventional MRI and DWI in 162 patients with homogeneous thyroid micronodules confirmed by operation and pathology. Of 226 homogeneous thyroid micronodule lesions, 47 were benign and 179 were malignant. The imaging data of all lesions were analyzed statistically. Results among the 47 benign nodules confirmed by postoperative pathology, 13 had high signal DWI, 32 had equal signal intensity and 2 had low signal intensity, 135 had high signal DWI, 36 had equal signal intensity and 8 had low signal intensity in 179 malignant nodules. 鍙朾鍊間負0 s/mm~2鏃,
