【摘要】：目的:觀察負(fù)壓封閉引流聯(lián)合"清筋疽散"沖洗治療糖尿病足潰瘍臨床療效。方法:收集糖尿病足潰瘍患者72例,隨機(jī)將其分為兩組,對(duì)照組35例予單純負(fù)壓封閉引流組,觀察組37例予負(fù)壓封閉引流聯(lián)合"清筋疽散"沖洗治療,連續(xù)治療4周。比較兩組患者創(chuàng)面愈合率、細(xì)菌清除率、肉芽組織覆蓋率、肉芽組織生長(zhǎng)厚度及引流組織中相關(guān)因子類胰島素一號(hào)增長(zhǎng)因子(IGF-1)、表皮生長(zhǎng)因子(EGF)、腫瘤壞死因子α(TNF-α)、一氧化氮(nitricoxide,NO)含量變化,并評(píng)價(jià)兩種療法的臨床療效。結(jié)果:治療4周后,與對(duì)照組比較,觀察組患者潰瘍創(chuàng)面處肉芽組織覆蓋率、肉芽組織生長(zhǎng)厚度、細(xì)菌清除率及創(chuàng)面愈合率均顯著升高(P0.05),引流組織中相關(guān)因子IGF-1、EGF及NO的含量顯著升高(P0.05),TNF-α的含量顯著降低(P0.05);且觀察組的總有效率顯著升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:VSD聯(lián)合"清筋疽散"沖洗治療糖尿病病足潰瘍可有效促進(jìn)肉芽組織生長(zhǎng)及潰瘍組織創(chuàng)面的修復(fù)、減輕創(chuàng)面感染幾率及炎性反應(yīng),從整體上提高創(chuàng)面愈合率,臨床效果較優(yōu),該療法可以在臨床上進(jìn)行推廣。
[Abstract]:Objective: to observe the clinical effect of negative pressure sealing drainage combined with Qingjingangrene Powder in the treatment of diabetic foot ulcer. Methods: Seventy-two patients with diabetic foot ulcer were randomly divided into two groups: the control group (n = 35) and the control group (n = 35), and the observation group (n = 37) were treated with negative pressure sealing drainage combined with "Qingjinjiangsan" for 4 weeks. The wound healing rate, bacterial clearance rate and granulation tissue coverage were compared between the two groups. The changes of granulation tissue growth thickness and the contents of IGF-1, EGFN, TNF- 偽, nitric oxide (no) in the drainage tissues were evaluated, and the clinical effects of the two treatments were evaluated. Results: after 4 weeks of treatment, compared with the control group, the granulation tissue coverage rate and granulation tissue growth thickness in the treatment group were compared with those in the control group. The bacterial clearance rate and wound healing rate were significantly increased (P 0.05), and the contents of IGF-1 EGF and no in drainage tissues were significantly increased. The content of P0.05 TNF- 偽 decreased significantly, and the total effective rate of the observation group was significantly higher than that of the control group (P 0.05). Conclusion the treatment of foot ulcers in diabetic patients with foot ulcers by WW VSD combined with "Qingjinzhijian Powder" can effectively promote the growth of granulation tissue and the repair of ulcer tissue wounds, reduce the infection probability and inflammatory reaction of the wounds, and improve the healing rate of the wounds as a whole, and the clinical effect is better. This therapy can be popularized in clinic.
