【摘要】：目的探討住院糖尿病足潰瘍(DFU)患者截肢的相關(guān)因素,為降低DFU截肢風(fēng)險(xiǎn)提供理論依據(jù)。方法 2013年10月至2015年12月收治于該院的307例DFU患者,按是否接受截肢手術(shù)分為截肢組(28例)和非截肢組(279例),對兩組患者的臨床特征及實(shí)驗(yàn)室指標(biāo)進(jìn)行單因素分析,并采用多因素非條件Logistic回歸分析探討截肢的相關(guān)因素。結(jié)果截肢組患者的住院時(shí)間、超敏C反應(yīng)蛋白(hs-CRP)及纖維蛋白原(FIB)水平均明顯高于非截肢組(P0.05),血紅蛋白(Hb)、高密度脂蛋白(HDL)及血清清蛋白(ALB)水平明顯低于非截肢組(P0.05)。兩組患者不同Wagner分級截肢情況分布比較差異也有統(tǒng)計(jì)學(xué)意義(P=0.000)。多因素非條件Logistic回歸分析發(fā)現(xiàn)住院時(shí)間(OR=1.024;95%CI:1.009~1.039),Wagner分級(OR=2.779;95%CI:1.753~4.404)是DFU患者截肢的獨(dú)立危險(xiǎn)因素,HDL是DFU截肢的保護(hù)性因素(OR=0.161;95%CI:0.036~0.729)。結(jié)論 DFU患者Wanger分級越高,住院時(shí)間越長,截肢風(fēng)險(xiǎn)越高,HDL是DFU截肢的保護(hù)性因素。
[Abstract]:Objective to investigate the related factors of amputation of diabetic foot ulcer (DFU) patients in order to reduce the risk of DFU amputation. Methods from October 2013 to December 2015, 307 patients with DFU were divided into amputation group (n = 28) and non-amputation group (n = 279). Multivariate non-conditional Logistic regression analysis was used to study the related factors of amputation. Results in the amputation group, the levels of hypersensitive C-reactive protein hs-CRP (Hs-CRP) and fibrinogen (FIBs) were significantly higher than those in the non-amputation group (P 0.05), and the levels of HB, HDL and ALB in the amputation group were significantly lower than those in the non-amputation group (P 0.05). The distribution of amputation in different Wagner grades was also significantly different between the two groups (P < 0. 000). Multivariate conditional Logistic regression analysis showed that the length of stay in hospital was 1.009 (1.009) and 1.039 (Wagner grade) was an independent risk factor for amputation of DFU patients. It was the protective factor of DFU amputation. Conclusion the higher the Wanger grade, the longer the length of hospitalization and the higher the risk of amputation in DFU patients are the protective factors of DFU amputation.
