【摘要】：目的:檢測(cè)狼瘡腎炎(lupus nephritis,LN)患者血清和腎組織的白介素17(Interleukin 17,IL-17)表達(dá)水平,結(jié)合臨床相關(guān)指標(biāo),分析IL-17水平與疾病活動(dòng)度、腎臟病理?yè)p害程度等的相關(guān)性,初步探討IL-17在LN發(fā)病與發(fā)展中的作用。方法:采用免疫組化法檢測(cè)54例LN患者腎組織IL-17表達(dá)水平,酶聯(lián)免疫吸附測(cè)定法(enzyme-linked immunosorbent assay,ELISA)檢測(cè)26例LN患者和30例年齡、性別均匹配的健康對(duì)照者血清IL-17水平,并對(duì)結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:1.Ⅳ型LN患者腎組織的IL-17表達(dá)水平明顯高于Ⅲ型、Ⅴ型、Ⅲ+Ⅴ型、Ⅳ+Ⅴ型,差異有統(tǒng)計(jì)學(xué)意義。2.LN患者腎組織IL-17表達(dá)水平與腎臟病理急性指數(shù)(r=0.547,P0.001)、尿蛋白(r=0.446,P=0.001)、24h尿蛋白定量(r=0.399,P=0.003)呈正相關(guān),與腎臟慢性指數(shù)無相關(guān)性。3.LN患者血清IL-17水平高于健康對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義。4.根據(jù)疾病活動(dòng)指數(shù)將LN組分為L(zhǎng)N活動(dòng)組(11例)、LN非活動(dòng)組(15例),LN活動(dòng)組血清IL-17水平明顯高于LN非活動(dòng)組和健康對(duì)照組,LN非活動(dòng)組IL-17水平高于健康對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義。5.LN患者血清IL-17水平與疾病活動(dòng)指數(shù)呈正相關(guān)(r=0.640,P0.001),與補(bǔ)體C3水平呈負(fù)相關(guān)(r=-0.410,P=0.0026),與補(bǔ)體C4無相關(guān)性。結(jié)論:IL-17在LN的發(fā)病和發(fā)展中發(fā)揮一定作用。
[Abstract]:Objective: to detect the expression of interleukin-17 (IL-17) in serum and renal tissue of patients with lupus nephritis (LN), and to analyze the correlation between the level of IL-17 and disease activity and the degree of renal pathological damage.To explore the role of IL-17 in the pathogenesis and development of LN.Methods: the expression of IL-17 in kidney tissue of 54 LN patients was detected by immunohistochemistry, and the serum IL-17 level of 26 LN patients and 30 healthy controls matched by age and sex was detected by enzyme linked immunosorbent assay (Elisa).The results were analyzed statistically.Results the expression of IL-17 in renal tissue of LN patients with type 鈪，
