【摘要】：目的探討HbA1c水平與腦出血合并糖尿病患者遲發(fā)性腦水腫的相關(guān)性。方法回顧性分析115例于唐山市工人醫(yī)院神經(jīng)外科住院的腦出血合并糖尿病患者。采用HPLC法檢測HbA1c,根據(jù)HbA1c水平分為HbA1c7.0%組(A組)、7.0%~9.0%組(B組)和HbA1c9.0%組(C組),分析HbA1c水平對腦出血合并糖尿病患者遲發(fā)性腦水腫的影響。結(jié)果 3組遲發(fā)性腦水腫發(fā)生率差異有統(tǒng)計學(xué)意義(P0.05);腦出血合并糖尿病發(fā)生遲發(fā)性腦水腫患者水腫體積與HbA1c(r=0.814,P=0.000)、GCS評分(r=-0.780,P=0.000)、NIHSS評分(r=0.696,P=0.000)均相關(guān);多元線性回歸分析后,HbA1c與患者水腫體積仍相關(guān)(β=0.741,t=5.280,P=0.000)。結(jié)論 HbA1c水平與腦出血合并糖尿病患者遲發(fā)性腦水腫嚴重程度相關(guān)。
[Abstract]:Objective to investigate the correlation between HbA1c level and delayed cerebral edema in patients with cerebral hemorrhage and diabetes mellitus.Methods 115 cases of intracerebral hemorrhage complicated with diabetes mellitus in neurosurgery department of workers' hospital in Tangshan City were analyzed retrospectively.HbA1cwas detected by HPLC method. According to the level of HbA1c, HBA 1c7.0% group was divided into group A (7.0%) and group B (9.0%) and group C (9.0%). The effect of HbA1c level on delayed cerebral edema in patients with cerebral hemorrhage complicated with diabetes was analyzed.Results there were significant differences in the incidence of delayed cerebral edema among the three groups (P 0.05), and the volume of edema in patients with cerebral hemorrhage complicated with diabetes mellitus was correlated with the GCS score of HBA _ 1C _ (0.14) P _ (0.000) and the NIHSS score (r _ (0.696) P _ (0.000)).After multivariate linear regression analysis, HbA1c was still correlated with the edema volume (尾 -0.741t = 5.280).Conclusion the level of HbA1c is related to the severity of delayed cerebral edema in patients with cerebral hemorrhage and diabetes mellitus.
