百色市壯族血尿酸水平與代謝綜合征各組分及脂肪因子ZAG的相關(guān)性研究

【摘要】:目的:本研究以廣西百色市壯族人群作為研究對(duì)象,初步探討壯族人群的尿酸水平與MS各組分(高血脂、高血壓、高血糖、肥胖)及脂肪因子ZAG的相關(guān)性。通過(guò)構(gòu)建高尿酸血癥小鼠模型,進(jìn)一步論證SUA與ZAG的相關(guān)性。為預(yù)防和控制慢性代謝性疾病提供科學(xué)依據(jù),為HUA的防治提出新的方向。方法:1百色市壯族高尿酸血癥患者的生化指標(biāo)與代謝綜合征各組分的相關(guān)性分析(1)抽取百色市所轄的右江區(qū)、田東縣、田陽(yáng)縣、德?h、田林縣、靖西縣等地的市縣區(qū)醫(yī)院、右江民族醫(yī)學(xué)院直屬附屬醫(yī)院及附屬西南醫(yī)院體檢者中,三代及三代以上均為壯族的人群進(jìn)行調(diào)查;(2)對(duì)研究對(duì)象一般健康指標(biāo)及生化指標(biāo)進(jìn)行檢測(cè);(3)根據(jù)所測(cè)的一般健康指標(biāo)及生化指標(biāo),將受試者合并的代謝性疾病分為:肥胖、高血壓、高血脂、高血糖。2百色市壯族尿酸水平與新脂肪因子ZAG的相關(guān)性分析(1)從第一部分2284例受試者中,隨機(jī)抽取高尿酸血癥患者150例,健康對(duì)照組受試者150例。經(jīng)檢查均無(wú)心、肝、肺、腎、腦等臟器疾病,無(wú)消化道和血液系統(tǒng)疾病史。年齡范圍20-70歲。2組對(duì)象性別、年齡等基線資料比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);(2)實(shí)時(shí)熒光定量PCR檢測(cè)受試者全血ZAG mRNA表達(dá)水平;(3)ELISA法檢測(cè)受試者血清ZAG蛋白濃度。3基于高尿酸血癥小鼠模型探討血尿酸與ZAG的相關(guān)性(1)尿酸酶抑制法構(gòu)建高尿酸小鼠模型;(2)檢測(cè)小鼠血清生化指標(biāo)及ZAG蛋白濃度。結(jié)果:(1)二分類(lèi)Logistics回歸分析:代謝綜合征危險(xiǎn)組分(超重/肥胖、高血壓、高血糖、高血脂)是壯族高尿酸血癥的危險(xiǎn)因素(P0.05或P0.001),是否合并以上各危險(xiǎn)組分診斷高尿酸血癥的ROC曲線下面積與0.5相比,差異有統(tǒng)計(jì)學(xué)意義(P0.001);(2)HUA組血清中ZAG的表達(dá)水平高于對(duì)照組(P0.001),且ZAG mRNA的相對(duì)表達(dá)水平也較高(P0.001);SUA與ZAG呈正相關(guān)(P0.01);ZAG診斷HUA的ROC曲線下面積為0.770,與0.5相比差異有統(tǒng)計(jì)學(xué)意義(P0.001);(3)小鼠SUA與BUN,CREA,ZAG,FPG,TC,TG,LDL-C呈正相關(guān)(P0.05或P0.001),與HDL-C呈負(fù)相關(guān)(P0.001);SUA與ZAG的一元線性回歸方程為:Y(SUA)=-239.758+2.533X(ZAG)。結(jié)論:(1)壯族HUA受試者合并MS各組分的順序依次為:高血脂,高血壓,高血糖,肥胖;各組分也是壯族HUA的危險(xiǎn)因素,是否合并各危險(xiǎn)組分對(duì)壯族高尿酸血癥的診斷效果欠佳,靈敏度較低;(2)壯族人群尿酸水平與ZAG呈正相關(guān),血清ZAG濃度對(duì)高尿酸血癥的診斷準(zhǔn)確性一般;(3)尿酸酶抑制法可以成功誘導(dǎo)高尿酸小鼠模型;基于動(dòng)物模型進(jìn)一步驗(yàn)證了尿酸水平與ZAG呈正相關(guān)。
[Abstract]:Objective: to investigate the correlation between uric acid level and MS components (hyperlipidemia, hypertension, hyperglycemia, obesity) and fat factor ZAG in Zhuang population in Baise, Guangxi.The relationship between SUA and ZAG was further demonstrated by constructing hyperuricemia mouse model.To provide a scientific basis for the prevention and control of chronic metabolic diseases, and to provide a new direction for the prevention and treatment of HUA.Methods the correlation analysis of biochemical indexes and metabolic syndrome components in Zhuang patients with hyperuricemia in 1: 1) the hospitals in Youjiang District, Tiandong County, Tianyang County, Debao County, Tianlin County, Jingxi County, etc., which are under the jurisdiction of Baise City, were selected from the city and county hospitals of Baise City, such as Youjiang District, Tiandong County, Tianyang County, Debao County, Tianlin County, Jingxi County, etc.Among those under the affiliated Hospital of Youjiang Medical College for nationalities and the affiliated Southwest Hospital,A survey of the population of Zhuang nationality of the third generation and more than three generations was carried out. (2) the general health index and biochemical index of the subjects were tested. The serum ZAG protein concentration was detected by Elisa. 3 based on the model of hyperuricemia mice, the relationship between serum uric acid and serum levels of serum ZAG protein was studied.Correlation of ZAG 1) the hyperuricemia mouse model was established by uric acid enzyme inhibition method. The serum biochemical indexes and the concentration of ZAG protein in mice were detected.Results Logistics regression analysis showed that the risk components of metabolic syndrome (overweight / obesity, hypertension, hyperglycemia),Hyperlipidemia) is a risk factor of hyperuricemia in Zhuang nationality (P 0.05 or P 0.001).There was a significant difference in the expression of ZAG in serum of P0.001HUA group compared with the control group, and the relative expression level of ZAG mRNA was also higher than that of the control group. There was a positive correlation between P0.001SUA and ZAG. The area under the ROC curve of the diagnosis of HUA by ZAG was 0.770, which was significantly higher than that of the control group.SUA was positively correlated with TGG LDL-C (P0.05 or P0.001C), and negatively correlated with HDL-C (P0.001SUA and ZAG). The linear regression equation between SUA and ZAG was: yam-SUA-239.758 2.533XZAGG.Conclusion (1) the sequence of MS components in Zhuang HUA subjects was as follows: hyperlipidemia, hypertension, hyperglycemia, obesity, components were also the risk factors of HUA in Zhuang nationality, the diagnostic effect of hyperuricemia in Zhuang was not good.There was a positive correlation between uric acid level and ZAG in Zhuang nationality population. The diagnostic accuracy of serum ZAG concentration for hyperuricemia was 3%) the mice model of hyperuricemia could be induced successfully by the method of inhibition of uric acid enzyme.The positive correlation between uric acid level and ZAG was further verified based on animal model.
