【摘要】：目的分析累及髖關(guān)節(jié)的痛風(fēng)性關(guān)節(jié)炎的臨床相關(guān)資料,提高對(duì)該疾病的認(rèn)識(shí)和診治能力。方法回顧性分析2014年1月至2017年3月在我院就診的痛風(fēng)性關(guān)節(jié)炎的病例,根據(jù)是否累及髖這個(gè)特殊部位分為髖關(guān)節(jié)組和常見關(guān)節(jié)組(髖關(guān)節(jié)不累及),其中髖關(guān)節(jié)組進(jìn)一步根據(jù)病情分為急性組和慢性組,比較各組病例的基本臨床資料。結(jié)果與常見關(guān)節(jié)組相比,髖關(guān)節(jié)組病例通常合并肥胖或既往患側(cè)髖部外傷史,男性患者發(fā)病年齡輕,同時(shí)髖關(guān)節(jié)組病例全部為單側(cè)髖部受累,局部無明顯紅腫熱痛。CT及超聲檢查可為早期診斷提供輔助,必要時(shí)需手術(shù)或病理檢查確診。結(jié)論累及髖關(guān)節(jié)的痛風(fēng)性關(guān)節(jié)炎臨床上缺乏特異診斷標(biāo)準(zhǔn),容易與髖部其他相關(guān)疾病混淆,在髖部疾病的臨床診斷不明確時(shí),應(yīng)考慮髖關(guān)節(jié)痛風(fēng)性關(guān)節(jié)炎的可能性。
[Abstract]:Objective to analyze the clinical data of gouty arthritis involving hip joint and improve the ability of diagnosis and treatment.Methods the cases of gouty arthritis in our hospital from January 2014 to March 2017 were analyzed retrospectively.The patients were divided into hip group and common joint group according to whether the hip was involved or not. The hip joint group was further divided into acute group and chronic group according to the condition. The basic clinical data of each group were compared.Results compared with the common joint group, the hip group was usually associated with obesity or previous history of hip trauma, and the male patients had a younger onset, while the hip group had unilateral hip involvement.No obvious redness, swelling and pain. Ct and ultrasound can provide assistance for early diagnosis. If necessary, the diagnosis should be confirmed by surgery or pathology.Conclusion the gouty arthritis involving hip joint lacks specific diagnostic criteria and is easily confused with other hip related diseases. The possibility of hip gouty arthritis should be considered when the clinical diagnosis of hip disease is not clear.
