【摘要】：目的探討類風(fēng)濕關(guān)節(jié)炎(RA)生物制劑治療期間中性粒細胞淋巴細胞比率[中性粒細胞與淋巴細胞比值(NLR)]的變化及其與關(guān)節(jié)疼痛和關(guān)節(jié)功能恢復(fù)情況的關(guān)系。方法以RA患者66例為RA組,采用抗風(fēng)濕藥物和生物制劑益賽普治療,3個月為1療程,患者均治療1個療程以上,治療后均隨訪至少3個月。另選取30例性別、年齡匹配的同期健康查體志愿者為對照組。RA組治療前后和對照組入選次日均取靜脈血2 m L進行血細胞分析檢測比較兩組NLR。統(tǒng)計RA組治療前、治療后和隨訪3個月的關(guān)節(jié)疼痛視覺模擬評分(VAS)和關(guān)節(jié)功能分級。分析RA生物制劑治療期間NLR評估與其治療后3個月關(guān)節(jié)疼痛和關(guān)節(jié)功能恢復(fù)情況的關(guān)系及其對關(guān)節(jié)疼痛和關(guān)節(jié)功能恢復(fù)不良的預(yù)測價值。結(jié)果與對照組比較,RA組治療前的NLR升高(P0.05)。與治療前比較,RA組治療后的NLR降低(P0.05)。與RA組隨訪3個月VAS≥4分患者比較,其VAS4分患者治療前后的NLR降低(P0.05)。與RA組隨訪3個月關(guān)節(jié)功能為Ⅱ級和Ⅲ級患者比較,其隨訪3個月關(guān)節(jié)功能為Ⅰ級患者治療前后的NLR降低(P0.05)。Spearman無條件相關(guān)分析結(jié)果顯示,RA生物制劑治療前后的NLR與訪3個月的VAS評分及關(guān)節(jié)功能分級均呈正相關(guān)。ROC曲線分析結(jié)果顯示,RA生物制劑治療前后的NLR評估其隨訪3個月的VAS評分及關(guān)節(jié)功能分級的價值良好,其中以治療后的NLR評估其隨訪3個月的VAS評分及關(guān)節(jié)功能分級的價值較好。結(jié)論 RA生物制劑治療可降低患者NLR,其NLR與治療后關(guān)節(jié)疼痛及關(guān)節(jié)功能恢復(fù)情況均相關(guān),且對其關(guān)節(jié)疼痛和關(guān)節(jié)功能恢復(fù)情況預(yù)測價值良好,NLR可能作為RA生物制劑治療患者關(guān)節(jié)疼痛和關(guān)節(jié)功能恢復(fù)情況評估的參考指標。
[Abstract]:Objective to investigate the changes of neutrophil lymphocyte ratio (neutrophil to lymphocyte ratio) during the treatment of rheumatoid arthritis (RA) and its relationship with joint pain and joint function recovery. 66 patients with RA were in RA group. The patients were treated with antirheumatic drugs and biologic agents Isop for a course of 3 months or more. All patients were followed up for at least 3 months after treatment. 30 cases of sex were selected. The age matched healthy volunteers of the same period were the control group. Before and after treatment and the control group, the venous blood was taken from 2 mL of vein blood before and after treatment, and the blood cell analysis was made between the two groups. Before and after treatment, NLRs of the two groups were analyzed and compared. Visual analogue score of joint pain (VAS) and joint function grading were evaluated after treatment and 3 months follow-up. The relationship between NLR evaluation during RA biologic therapy and joint pain and recovery of joint function at 3 months after treatment was analyzed. Results compared with the control group, the NLR of RA group increased before treatment, and the NLR of RA group decreased after treatment compared with that before treatment.
