【摘要】：目的:探討米諾環(huán)素對中重度慢性牙周炎合并2型糖尿病患者的臨床指標(biāo)、炎癥因子和血糖的影響。方法:選取我院2013年2月-2015年8月收治的中重度慢性牙周炎合并2型糖尿病患者125例,按照隨機(jī)數(shù)字表法分為對照組(63例)和觀察組(62例)。兩組患者均給予潔治術(shù)和根面平整術(shù)等牙周基礎(chǔ)治療;對照組患者術(shù)后給予碘甘油注滿牙周袋,2~4次/d;觀察組患者術(shù)后給予鹽酸米諾環(huán)素軟膏注滿牙周袋,每周1次。兩組患者均治療4周。觀察兩組患者治療前和治療后3個月的臨床指標(biāo)[探診深度、臨床附著水平(CAL)和探診出血率]、血清炎癥因子(C反應(yīng)蛋白和腫瘤壞死因子α)、血糖(空腹血糖和糖化血紅蛋白)水平,并記錄不良反應(yīng)發(fā)生情況。結(jié)果:治療前,兩組患者臨床指標(biāo)、血清炎癥因子和血糖水平比較,差異均無統(tǒng)計學(xué)意義(P0.05)。治療后3個月,兩組患者探診深度、探診出血率和血清炎癥因子水平均明顯降低,且觀察組明顯低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05);兩組患者CAL均較治療前增加,但治療前后比較、組間比較差異均無統(tǒng)計學(xué)意義(P0.05);兩組患者空腹血糖均明顯降低,差異有統(tǒng)計學(xué)意義(P0.05),但組間比較差異無統(tǒng)計學(xué)意義(P0.05);對照組患者糖化血紅蛋白水平較治療前降低,但差異無統(tǒng)計學(xué)意義(P0.05);觀察組患者糖化血紅蛋白水平明顯降低,且觀察組明顯低于對照組,差異有統(tǒng)計學(xué)意義(P0.05)。觀察組患者不良反應(yīng)發(fā)生率(3.23%)與對照組(0)比較,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:米諾環(huán)素治療中重度慢性牙周炎合并2型糖尿病可有效改善患者牙周炎的臨床指標(biāo),降低血清炎癥因子和血糖水平,且安全性較高。
[Abstract]:Objective: to study the clinical indexes of minocycline in patients with moderate and severe chronic periodontitis complicated with type 2 diabetes mellitus. Methods: 125 patients with moderate and severe chronic periodontitis complicated with type 2 diabetes mellitus from February 2013 to August 2015 were selected. The patients in the two groups were divided into two groups: control group (n = 63) and observation group (n = 62). The patients in the control group were given iodine glycerol to fill the periodontal bag for 4 times per day, and the patients in the observation group were given minocycline hydrochloride ointment to fill the periodontal bag after operation. Once a week, both groups were treated for 4 weeks. The clinical indexes before and 3 months after treatment in both groups were observed. Clinical adhesion level (CAL) and diagnostic bleeding rate], serum inflammatory factor C reactive protein (CRP) and tumor necrosis factor alpha (TNF- 偽), blood glucose (fasting blood glucose and glycosylated hemoglobin), and the occurrence of adverse reactions were recorded. There was no significant difference in clinical indexes, serum inflammatory factors and blood glucose levels between the two groups. After 3 months of treatment, the depth, bleeding rate and serum inflammatory factor level of the two groups were significantly lower than those of the control group. The CAL of the two groups was significantly higher than that of the control group, but there was no significant difference between the two groups before and after treatment, and the fasting blood glucose in the two groups was significantly lower than that in the control group, and there was no significant difference between the two groups before and after treatment, and there was no significant difference between the two groups before and after treatment. The difference was statistically significant (P 0.05), but there was no significant difference between the two groups (P 0.05), the level of glycosylated hemoglobin in the control group was lower than that before treatment, but the difference was not statistically significant (P 0.05), the level of glycosylated hemoglobin in the observation group was significantly lower than that in the control group. The incidence of adverse reactions in the observation group was 3.23 (P < 0.05), which was significantly lower than that in the control group (P 0.05). Conclusion: minocycline in the treatment of moderate and severe chronic periodontitis combined with type 2 diabetes mellitus can effectively improve the clinical indicators of periodontitis, reduce the level of serum inflammatory factors and blood sugar, and the safety is higher.
