【摘要】：目的:觀察益腎化濕顆粒聯(lián)合舒洛地特對早期老年糖尿病腎病(DN)氧化應激及內(nèi)皮功能的影響。方法:86例早期糖尿病腎病患者隨機分成兩組,兩組均給予常規(guī)治療,對照組加用舒洛地特注射液肌肉注射600 LSU(酯酶單位),1次·d~(-1),連續(xù)治療2周后,口服舒洛地特軟膠囊,1粒/次,2次/d,連續(xù)治療4個月。觀察組在對照組治療的基礎上加用益腎化濕顆粒,10 g·次-1,3次/d。連續(xù)治療4個月。觀察治療前后血糖,血肌酐(SCr),超氧化物歧化酶(SOD),丙二醛(MDA),血管性血友病因子(v WF),內(nèi)皮素(ET)-1以及24 h尿白蛋白排泄率(24 h UAER)。結(jié)果:與治療前比較,兩組24 h UAER和SCr水平明顯下降(P0.05,P0.01),尤其觀察組更加明顯[分別為(72.3±15.8)mg·(24 h)-1,(65.8±20.6)μmol·L~(-1)],與對照組[分別為(87.6±17.2)mg·(24 h)-1,(91.6±23.8)μmol·L~(-1)]比較差異有統(tǒng)計學意義(均P0.05)。治療后觀察組MDA,v WF,ET-1[分別為(4.01±0.4)μmol·L~(-1),(159.5±26.4)%,(55.7±7.2)ng·L~(-1)]較治療前明顯下降(均P0.05),與對照組[分別為(5.31±0.51)μmol·L~(-1),(181.3±28.2)%,(60.4±7.5)ng·L~(-1)]比較差異有統(tǒng)計學意義(P0.05)。治療后觀察組SOD[(129.3±7.1)U·m L~(-1)]較治療前明顯上升(P0.05),與對照組[(70.2±4.5)U·m L~(-1)]比較差異有統(tǒng)計學意義(P0.05)。結(jié)論:益腎化濕顆粒具有較強抗氧化作用,能有效的延緩血管內(nèi)皮的損傷,減少蛋白尿的排出,而且聯(lián)合舒洛地特共同治療,調(diào)節(jié)血管內(nèi)皮功能的效果更加明顯,延緩早期DN發(fā)展。
[Abstract]:Objective: to observe the effect of Yishen Huashi granule combined with sulodide on oxidative stress and endothelial function in early aged diabetic nephropathy patients. Methods: 86 patients with early diabetic nephropathy were randomly divided into two groups. In the control group, sulodite injection was injected intramuscularly into the control group with 600 LSUA (Esterase unit) once a day, and after 2 weeks of continuous treatment, The patients in the observation group were treated with 10 g of Yishen Huashi granule (10 g / d) and 3 times / d for 4 months. The blood glucose was observed before and after treatment. Serum creatinine, superoxide dismutase (SOD), malondialdehyde (MDA), von Willebrand factor (VHD), endothelin (et) -1 and 24 h urinary albumin excretion rate (24 h UAERA) were compared with those before treatment. The levels of UAER and SCr in the two groups decreased significantly at 24 h, especially in the observation group (72.3 鹵15.8)mg 24 h, 65.8 鹵20.6 渭 mol / L), and were significantly higher than those in the control group [87.6 鹵17.2)mg 24 h, 91.6 鹵23.8) 渭 mol L ~ (-1)] (P0.05). After treatment, MDA-vWFET-1 [4.01 鹵0.4 渭 mol] was significantly higher than that of the control group (P < 0.05).
