【摘要】：目的探討Roux-en-Y消化道重建術(shù)對胃癌合并2型糖尿病患者血糖的影響。方法回顧性分析該院于2010年—2014年6月收治的52例以Roux-en-Y消化道重建行手術(shù)治療的胃癌合并2型糖尿病患者,比較術(shù)前、術(shù)后1、3、6個月的空腹血糖、餐后2 h血糖、糖化血紅蛋白(HbA1c)、體重指數(shù)(BMI)的變化情況,密切關(guān)注術(shù)后的降糖藥物使用情況。結(jié)果Roux-en-Y消化道重建術(shù)術(shù)后所有患者的空腹及餐后血糖均較術(shù)前逐漸降低(P0.05),伴隨著術(shù)后血糖的改善,HbA 1c較術(shù)前逐漸下降(P0.05);患者術(shù)后各時間段BMI較術(shù)前均無明顯變化(PO.05)。52例患者中,8例(15.3%)術(shù)后出院時血糖達(dá)正常水平,停用降糖藥物;17例(32.7%)于術(shù)后3個月內(nèi)停藥;9例(17.3%)于術(shù)后3~6個月停藥;18例(34.6%)于術(shù)后6個月仍在使用降糖藥物,但用藥劑量均有不通程度的減少,目前仍在隨訪。結(jié)論 Roux-en-Y消化道重建術(shù)是治療2型糖尿病的有較方法。
[Abstract]:Objective to investigate the effect of Roux-en-Y alimentary tract reconstruction on blood glucose in patients with gastric cancer complicated with type 2 diabetes mellitus. Methods 52 cases of gastric cancer complicated with type 2 diabetes mellitus treated with Roux-en-Y gastrointestinal reconstruction from 2010 to June 2014 were retrospectively analyzed. The changes of fasting blood glucose (FBG), postprandial blood glucose (2 h), glycosylated hemoglobin (HbA1cU), and body mass index (BMIs) were compared before and after 1, 3 and 6 months postoperatively. Results the fasting and postprandial blood glucose of all patients after Roux-en-Y alimentary tract reconstruction was decreased gradually compared with that before operation (P 0.05), and with the improvement of blood glucose after operation, HbA1c decreased gradually compared with that before operation (P 0.05). There was no significant change in BMI at each time after operation compared with that before operation. Among 52 patients with PO.050.52 cases, 8 cases (15.3%) blood glucose reached normal level when discharged from hospital after operation. (17 cases stopped using hypoglycemic drugs 32.7%) within 3 months after operation, 9 cases (17.3%) and 18 cases (34.6 cases) were still using hypoglycemic drugs at 6 months after operation, but the dosage of these drugs were all decreased to a certain extent. Conclusion Roux-en-Y alimentary tract reconstruction is a better method for the treatment of type 2 diabetes mellitus.
