【摘要】：目的探討老年代謝綜合征(metabolic syndrome,MS)人群胱抑素C(cystatin C,Cys C)濃度及其與MS主要組分及胰島素敏感指數(homeostasis model assessment of insulin resistance,HOMA-IR)之間的關系。方法參加體檢的404例老年人(60歲)及60例無MS組分的非老年人(≤60歲)納入本研究,測定MS主要組分、空腹胰島素(fasting insulin,FINS)、Cys C、血清肌酐(serum creatinine,SCr)等指標;按MS診斷標準將老年人群分為MS組與非MS組,進一步按MS組分分組,分析不同組間Cys C差異及其與MS組分間的關系。結果研究人群Cys C性別差異無統(tǒng)計學意義(P0.05),MS組Cys C濃度顯著高于非MS組(1.10±0.25 vs 1.02±0.25,P0.01),隨著研究人群含有MS組分的增多,Cys C顯著增高(P0.05),其靈敏度優(yōu)于SCr及估算腎小球濾過率(estimated glomerular filtration rate,e GFR);多元逐步回歸分析表明體質量指數(body mass index,BMI)、三酰甘油(triglyceride,TG)、HOMA-IR是Cys C獨立影響因素。結論隨著研究人群MS組分數增加,Cys C顯著上升,可作為老年MS人群早期腎功能損傷及心血管疾病(cardiovascular disease,CVD)危險性評估的指標。
