【摘要】：【目的】本文主要觀察臨床上二甲雙胍、格列美脲、胰島素聯(lián)用安慰劑和分別聯(lián)合使用DPP-4抑制劑(沙格列汀)治療2型糖尿病的療效�！痉椒ā渴占�2型糖尿病患者60例,分為實(shí)驗(yàn)組和對照組,其中每小組均為10例。實(shí)驗(yàn)組為二甲雙胍(500mg/次,早晚各1次)+沙格列汀(5mg/日,早餐前)、格列美脲(2mg/日)+沙格列汀、胰島素(總量50IU-80IU/日)+沙格列汀分別為A組、B組、C組。對照組為二甲雙胍、格列美脲、胰島素分別與安慰劑聯(lián)用為D組、E組、F組。治療4周、12周后,觀察觀察組和對照組血糖、糖化血紅蛋白、體重指數(shù)、胰島素抵抗指數(shù)、胰島素分泌指數(shù)等指標(biāo)變化�！窘Y(jié)果】經(jīng)治療后,Met觀察組中Hb A1c、BMI、FPG、2h PG、HOMA-IR各指標(biāo)值下降顯著與對照組,HOMA-β指標(biāo)值升高顯著與對照組,均具有統(tǒng)計學(xué)意義(P0.05)。Gli觀察組中Hb A1c、FPG、2h PG指標(biāo)值下降均顯著于對照組,且HOMA-β值升高顯著于對照組,均具有統(tǒng)計學(xué)意義(P0.05)。Ins觀察組中Hb A1c、FPG、2h PG各指標(biāo)值均較對照組明顯降低,HOMA-β值明顯升高,均具有統(tǒng)計學(xué)意義(P0.05)�！窘Y(jié)論】Saxagliptin聯(lián)用Metformin、Glimepiride、Insulin治療2型糖尿病與Metformin、Glimepiride、Insulin聯(lián)用安慰劑相比,更能有效控制血糖,降低糖化血紅蛋白,不增加體重,改善胰島素抵抗和胰島β細(xì)胞功能。但聯(lián)用的遠(yuǎn)期療效、安全性、可耐受性、副作用小、低血糖發(fā)生率低等其他諸多特點(diǎn)還有待進(jìn)一步研究。
[Abstract]:[objective] to observe the clinical efficacy of metformin, glimepiride, insulin combined with placebo and DPP-4 inhibitor (salgletine) in the treatment of type 2 diabetes mellitus. Each group was divided into experimental group and control group. The experimental group was treated with metformin 500 mg / time, morning and evening) salgletine 5 mg / d, before breakfast, glimepiride 2 mg / d) salgletine. The insulin (total 50 IU-80 IUU / d) in group A, group B, group C, control group, metformin, glimepiride and insulin combined with placebo, respectively, were used in group D and group E, respectively. After 4 weeks and 12 weeks of treatment, blood glucose was observed in the observation group and the control group. [results] after treatment, the HbA1cA1cPMIFPGG 2 h PGHOMA-IR index decreased significantly and the control group increased HOMA- 尾 index significantly compared with the control group. The HbA1cFPGG 2h PG values in the observation group were significantly lower than those in the control group, and the values of HOMA- 尾 were significantly higher than those in the control group. The values of HbA1cFPGG 2 h PG in the observation group were significantly lower than those in the control group, and the values of HOMA- 尾 were significantly higher in the observation group than those in the control group, and the values of HOMA- 尾 in the observation group were significantly higher than those in the control group, and the values of HbA1cFPGG in the observation group were significantly higher than those in the control group. [conclusion] Saxagliptin combined with Metforminmin Glimepiride Insulin in the treatment of type 2 diabetes is more effective in controlling blood glucose, reducing glycosylated hemoglobin and not increasing body weight than that of Metforminmin Glimepiride Insulin in the treatment of type 2 diabetes. To improve insulin resistance and islet 尾 -cell function, but the long-term efficacy, safety, tolerance, less side effects, low incidence of hypoglycemia and many other characteristics need to be further studied.
