【摘要】：目的了解家庭醫(yī)生式健康管理服務(wù)對航天職工2型糖尿病(T2DM)患者相關(guān)健康指標的干預效果。方法以北京市某航天企業(yè)職工中300例T2DM患者為研究對象,實施家庭醫(yī)生式健康管理干預,觀察干預后24個月的空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血紅蛋白(Hb A1c)、甘油三酯(TG)、低密度脂蛋白膽固醇(LDL-C)、體重指數(shù)(BMI)等指標的變化情況。結(jié)果與干預前相比,干預期后患者FPG平均下降1.48 mmol/L,2 h PG平均下降2.42 mmol/L,Hb A1c平均下降0.38%,TG平均降低0.16 mmol/L,舒張壓(DBP)平均下降7.96 mm Hg,以上各項指標干預前后差異顯著(P0.05);患者收縮壓(SBP)和Hb A1c,LDL-C和Hb A1c干預后聯(lián)合達標率分別提升4.01%和5.33%,差異無統(tǒng)計學意義;SBP和LDL-C和Hb A1c干預后聯(lián)合達標率下降0.21%(P0.05);患者相關(guān)知識平均分干預后較干預前平均提升1.08分(P0.05);患者自我管理水平在飲食控制、規(guī)律運動、血糖監(jiān)測、足部護理和高低血糖處理五個維度上,干預后得分明顯高于干預前(P0.05)。結(jié)論家庭醫(yī)生式健康管理干預模式有效提升了航天職工中T2DM患者的血糖控制水平、相關(guān)健康知識和自我管理水平。
[Abstract]:Objective to investigate the intervention effect of family doctor health management service on related health indexes in aerospace workers with type 2 diabetes mellitus (T2DM). Methods 300 patients with T2DM in a certain aerospace enterprise in Beijing were studied. The implementation of family doctor-style health management intervention, The changes of fasting blood glucose (FPG) in 24 months after intervention, 2 hours of postprandial blood glucose and 2 hours of PGN, HbA1cP, TGN, LDL-C0 and BMIs were observed. The average FPG decreased by 1.48 mmol 路L ~ (-1) h PG decreased 2.42 mmol / L HbA1c decreased by 0.38 mmol 路L ~ (-1) TG decreased by 0.16 mmol 路L ~ (-1) and diastolic blood pressure decreased by 7.96 mm Hg.The above indexes were significantly different before and after intervention (P _ (0.05); SBP) and HB A _ (1) LDL-C and HB. After the intervention of A1c, the rate of reaching the standard was increased by 4.01% and 5.33, respectively. There was no significant difference between SBP and LDL-C and HbA1c after intervention, the rate of reaching the standard decreased by 0.21 and P0.05; the average score of patients' related knowledge increased by 1.08 scores and the average score of P0.05 after intervention; the self-management of patients. Level in diet control, The scores of regular exercise, blood glucose monitoring, foot nursing and hyperglycemia treatment were significantly higher after intervention than those before intervention (P 0.05). Conclusion Family doctor health management intervention model can effectively improve the blood glucose control level of T2DM patients in aerospace workers. Relevant health knowledge and self-management level.
