更年期女性膽固醇水平調(diào)查與影響因素分析

【摘要】:目的研究膽固醇代謝與絕經(jīng)的關(guān)聯(lián)和可能的影響因素。方法對杭州市拱墅區(qū)963例40~60歲女性進(jìn)行橫斷面調(diào)查,收集其月經(jīng)史及其他臨床資料,填寫改良Kupperman更年期癥狀量表(mKMI),檢測血清總膽固醇(TC)、低密度脂蛋白膽固醇(LDL-C)、卵泡刺激素(FSH)、黃體生成素(LH)、雌二醇(E_2)水平。收集參與者教育程度、婚姻狀態(tài)、職業(yè)狀態(tài)、收入、居住地等社會人口資料。研究不同絕經(jīng)狀態(tài)女性膽固醇水平及高膽固醇血癥、LDL-C升高的發(fā)生率,并分析年齡、絕經(jīng)狀態(tài)、FSH、E_2、社會人口等因素對血清膽固醇的影響。結(jié)果963例參與者平均年齡(51.0±5.6)歲,其中絕經(jīng)前期302例(31.4%),圍絕經(jīng)期197例(20.5%),絕經(jīng)后期464例(48.2%),絕經(jīng)后參與者的平均絕經(jīng)年齡(49.9±3.8)歲。在所有參與者中,絕經(jīng)前期mKMI總分(6.36±6.43)顯著低于圍絕經(jīng)期(10.30±7.88)和絕經(jīng)后期(10.35±7.97)(P0.001)。絕經(jīng)前后平均TC、LDL-C水平呈顯著上升趨勢,FSH、LH水平呈上升趨勢,E_2水平呈下降趨勢(P0.001)。所有參與者中高膽固醇血癥比例為13.3%,LDL-C升高比例為15.0%,從絕經(jīng)前期過渡到圍絕經(jīng)期、絕經(jīng)后期的過程中,高膽固醇血癥及LDL-C升高的發(fā)生率顯著上升(P0.001)。Logistic多元回歸分析表明FSH≥40IU/L是發(fā)生高膽固醇血癥和LDL-C升高的危險(xiǎn)因素(OR=2.821,95%CI=1.429~5.569,P=0.003;OR=2.587,95%CI=1.356~4.937,P=0.004),調(diào)整FSH水平后,年齡、絕經(jīng)狀態(tài)、E_2水平、社會人口因素與發(fā)生高膽固醇血癥和LDL-C升高均無顯著關(guān)聯(lián)(P0.05)。結(jié)論相比絕經(jīng)前期,圍絕經(jīng)期和絕經(jīng)后女性平均TC、LDL-C上升,膽固醇水平升高的發(fā)生率增加,FSH水平升高可能是膽固醇代謝紊亂的內(nèi)在機(jī)制,降低FSH水平可作為絕經(jīng)激素補(bǔ)充治療的目標(biāo)和療效指標(biāo)。
[Abstract]:Objective to study the relationship between cholesterol metabolism and menopause and its possible influencing factors. Methods 963 women aged 4060 years old in Gongshu District of Hangzhou City were investigated cross-sectional and their menstrual history and other clinical data were collected. Filling out the modified Kupperman climacteric symptom scale (MKMIA), measuring serum total cholesterol, low density lipoprotein cholesterol (LDL-C), follicle stimulating hormone (FSH), luteinizing hormone (LHH), estradiol (E2).
