血清VEGF及ICAM-1水平與糖尿病視網(wǎng)膜病變患者微血管損傷的關(guān)系

【摘要】:目的探討血清中血管內(nèi)皮生長(zhǎng)因子(VEGF)及細(xì)胞間黏附分子(ICAM)-1水平與糖尿病視網(wǎng)膜病變(DR)患者微血管損傷之間的關(guān)系。方法未應(yīng)用抗凝類藥物治療的2型糖尿病(T2DM)患者120例,根據(jù)臨床及眼底檢查確診將其分為非DR(NDR)組(n=56)、非增生型DR(NPDR)組(n=30)、增生型DR(PDR)組(n=34)。比較各組微血管病變發(fā)生率;采用流式細(xì)胞儀檢測(cè)內(nèi)皮細(xì)胞(ECs)、內(nèi)皮祖細(xì)胞(EPCs)計(jì)數(shù)百分比;采用酶聯(lián)免疫吸附法(ELISA)檢測(cè)各組血清中VEGF及ICAM-1水平。結(jié)果 PDR組糖尿病腎病(DN)及神經(jīng)病變的發(fā)生率明顯高于NDR組和NPDR組(均P0.05);與未發(fā)生微血管病變患者相比,發(fā)生微血管病變患者的EPCs水平明顯下降,而ECs、VEGF和ICAM-1水平均明顯升高(均P0.05);不管是否發(fā)生微血管病變,PDR組的ECs、VEGF和ICAM-1水平均高于NDR組和NPDR組,NPDR組均高于NDR組,PDR組EPCs水平低于NPDR組,NPDR組低于NDR組(均P0.05);PDR組VEGF、ICAM-1水平與ECs水平呈正相關(guān)(r=0.995,0.852,P0.05),與EPCs水平呈負(fù)相關(guān)性(r=-0.895,-0.920,P0.05)。結(jié)論 VEGF和ICAM-1在DR患者微血管損傷中高表達(dá),與ECs和EPCs密切相關(guān),提示VEGF和ICAM-1在DR患者微血管損傷形成中與血管內(nèi)皮功能密切相關(guān)。
[Abstract]:Objective to investigate the levels of vascular endothelial growth factor (VEGF), intercellular adhesion molecule (ICAM-1) and diabetic retinopathy (DRR) in serum. Methods 120 patients with type 2 diabetes mellitus (T2DM) without anticoagulant therapy were studied. According to the diagnosis of clinical and fundus examination, the patients were divided into two groups: non-DRN NDR group (n = 56) and non-proliferative DRN group (n = 30). The incidence of microvascular lesions in the proliferative DRV PDR group was compared. Flow cytometry was used to detect the percentage of ECsN and EPCs in endothelial progenitor cells. The levels of VEGF and ICAM-1 in serum of each group were detected by enzyme linked immunosorbent assay (Elisa). Results the serum levels of VEGF and ICAM-1 in PDR group were determined by diabetic nephropathy. The incidence of neuropathy in NDR group and NPDR group was significantly higher than that in NDR group and NPDR group (P 0.05). Compared with those without microvascular disease, the level of EPCs in patients with microvascular disease was significantly decreased, while the levels of ICAM-1 and ICAM-1 were significantly increased in patients with microvascular disease (all P 0.05). The levels of ICAM-1 and ICAM-1 in PDR group were higher than those in NDR group and NPDR group. The level of EPCs in PDR group was lower than that in NPDR group and that in NDR group was lower than that in NDR group (all P 0.05). In PDR group, the level of VEGF ICAM-1 was positively correlated with the level of ECs. There was a negative correlation between VEGF and EPCs level. Conclusion VEGF and ICAM-1 are highly expressed in Dr patients with microvascular injury. It is suggested that VEGF and ICAM-1 are closely related to vascular endothelial function in the formation of microvascular injury in Dr patients.
